First select "Size Threes", you will have to kick the lever and the bridge will rise. Just run past the flamer and through the door, you want to jump over the bar across the room (you should be able to slip through the enemies with minimal damage, if you go through the door on the right you will kill an enemy, the door after the bar is safe). Run straight up the stairs and to the right go through the door and to the right if you kick that door straight you can knock out a flamer (doesn't count as a kill) run across the bridge at the end of the room and kick the lever. then jump down and run towards the other end of the room to the left of the eye door is a "crawl space" that you can go in if you are wearing the size threes. once you are in and kick the first door the path is:

R,R,L,L,F,L,R,L,R,R,F,F,R,L,R,L,R,F,L

(R= Right, L= Left, F= Forward)

Kick open the eyeball door and you can exit the crawl space, jump down and go through the door at the end of the hall, keep moving forward and ignore all enemies go through that door. Turn right and avoid the two goons in that hallway, kick the first door on the left be careful not to accidentally kick the goon closest to that door and then you are home free.
