I almost quit because I couldn't figure out what I was doing wrong to unlock the "I'm sorry"-option, so I can totally understand you :D

This last achievement was driving me insane
If the diary is scrolled through completely and then EVERYONE survives, you unlock the “best ending” for Josh.
But you have to flip through the diary completely. There will be a page where Sam starts to speak, then you have to keep turning the pages. This is the only way it works.

I played from Ep 8 and still didnt get the "I'm sorry option"
Played from Ep 10, found the journal and Josh's head still got crushed
Just with that information Sam will inform Josh about Hannah & Beths "real misfortune" and so he will realise that the Wendigo is indeed Hannah. After that she spares him and with the correct answers made at the psychologist in Chapter 8 he will survive and regret his actions :)
Logically, I collected the diary on my first playthrough, but it's essential that it's collected again on every replay if it should have any effect on Josh's chances of survival.
