This character appears in various places, and you have to save her different times.

• First appearance.

She's at the very start of the Temple, hanging from a rope. You need to shoot or touch this button hanging at the side of the wall so a trap arrow cuts the rope and she's set free.



If you press the button that's hanging from the ceiling, she'll be harmed.

• Second appearance.

This one depends on your actions. To avoid getting trapped, go to the upper room where there's a lot of snakes and find this hidden place.



Then hit the cog until it breaks.

If you don't do this, you can get trapped there. If that happens, use a key to get out. If the Rival gets caged, break the cog in the upper room, then go back to her and use a key.

• Third appearance.

In the Forest zone she gets eaten by this plant close to a fountain, just destroy it to free her.

• Fourth appearance.

This time in the Crypt or Cementery, there's this secret passage in the room to the right of the zombie girls.



If you get there, you'll see a moving coffin. Destroy it to free her.

She can be found at the end of the path that's above this ship-shaped structure. You'll need the knife to get there.

• How to befriend: You have to avoid opening ANY treasure chest before getting to her boss fight. But you need the knife to get there, so you'll have to try this on NG+.
  
  As I'm informed in the comments, there's a secret path to get there without the knife. I haven't found it yet, so if I see it I'll update this guide.
  
  You can open the chest inside her house (Which only appears by following previous steps), and then start opening every other chest without any problem.

She can be found below this ship-shaped structure in the Forest area.

• How to befriend:
  
  You must survive a few minutes without harming any of the five plant in this screen:

Then go to her boss fight and she'll be there, happy to see you.

• How to befriend:
  
  You have to avoid harming snakes and find five of them caugh by walls and debris in the Temple zone, then rescue them with something like the knife, but be careful to not hit them.

I don't know where the fifth snake is, so you're on your own here.


She can be found at the left corner of the Crypt/Cementery zone, up in a relatively secret place that you can reach by using a rope or field mine.

• How to befriend:
  
  You have to find this little lost bat that's in one of the left corners of the Temple zone and catch it with the jar.

Then go to the Bat Queen location previously stated and drop that jar with the bat inside while crouching, this way you'll drop it calmly instead of throwing it and killing the bat inside.

You can't miss this one, she's inside the mansion that's in the Crypt/Cementery zone.

• How to befriend:

First off, there's two ways to befriend her. One way with and other without the glasses, which can be found in a secret place below the Crypt's fountain.

• If you have the glasses already:
  
  Enter her room in the Mansion and just wait without harming her during the first three attacks, then she'll start to mess around with you and then she'll scare you to death, literally. Then you'll have her heart.
  
  
• If you don't have the glasses:
  
  You'll need two items, the Cross and the Ankh, which will allow you to keep B pressed to hold the rosary while being in the same room as her. Wait a few seconds and you'll obtain her heart after getting instantly killed.

You can find her at the end of the Spider zone, which is fairly tiny. It can be found at the top of the vertical green colored bricks zone which is connected with the Forest and the Temple.

• How to befriend:

Passing through a check point you'll see a room with an altar and red spiders at the top. Just sleep in the altar for a few seconds and you'll be greeted with the Spider Queen.



If you're going for the Pacifist route you might want to avoid harming any spiders, however I did and got her heart anyways, it may be a bug.

She's at the bottom of the ocean, so you'll need the Shark suit.

• How to befriend:

Avoid harming any large eel, tiny fish or jellyfish. You can kill any red dragonfish or purple octopuses.

Then, kill the giant purple fish. Its corpse will appear somewhere else in the map, do not destroy it.



Also kill this specific octopus that's in front of this statue, and then a bigger creature will appear.



Now there will be four purple tentacles in different parts of the ocean, destroy them all and go back to where the Ocean Queen is. You might find the giant octopus corpse, do not destroy it.

She can be found in the Cementery/Crypt, it's the most pale one of all zombies. To fight here, you must kill the rest of the zombies.

• How to befriend:

First off, do not hit any zombie with Holy Water (using the jar in fountain water).

You need to keep breaking these large vases until one of them gives you this big meat.



Now catch it with a jar and throw it to the Zombie Queen, then she'll eat it and you'll be hugged by her. Press the directional keys to break out of the hug and now you have her heart.




This is the merchant you meet at the start of the game.

• How to befriend:

I got her heart in two different playthrough and by two different ways.

• Buying everything she has on sale.
  
  or
  
  
• Getting one million in coins and then coming back to her.

At this point you should have all their hearts.



There's only one heart left, the Demon Queen's heart.

To find her, you must go all the way to the left from the fountain in the Crypt/Cementery.

• How to befriend:

To have her heart, you must beat all the other queens (except for the Zombie Queen) in a duel using this item, which will not kill them or remove your relationship status if you beat them.



Then fight the Demon Queen with this very same item and if you beat her, she'll give you her heart!

The Angel Queen, named as "The Strongest One" by the community due to the achievement "Strongest on this island", can be found at the end of a secret passage in the Forest.



In this place (Third room from the right side of the Forest, where there's a lot of mushrooms) there's a hidden door passing through a chest and a vase in the upper side. Just throw a key there and you'll hear a door being opened.



In the middle of this secret area you'll see a chest and a few tiny flowers.



Catch a bit of water with your jar and throw it to the flowers, this way a mushroom will grow. Jump over it and it will take you to another secret passage.



Use the knife to get to the top of it and you'll find this statue.



To get into her boss fight, you first need to beat every single queen in the game, whether by killing them or befriending every queen and then using that item to beat them peacefully.

Then, a beam of light will appear here for you to be teleported to her domains.

• How to befriend:

It's not entirely clear right now, but it seems that she'll only give you her heart if you have found all treasures (a total of 100), gotten all trophies (a total of 7) and every other queen's hearts (a total of 11). Beat her on a duel with these conditions and you'll have the twelfth heart, Angel Queen's heart.
