True Ending
To unlock all achievements you will have to unlock the true ending, for that you will have to meet these requirements:
  • Reach Councillor Condidant Rank 9 before 18th November
  • Reach Justice Confidant Rank 8 before 24th November
  • Reach Faith Confidant Rank 5 before 18th December
  • Refuse all offers presented during interrogation
  • Refuse all offers presented in January and February

Special Move unlocked in the first palace. After knocking down an enemy you can attack again or pass to one of your team members.
Pass to one of them and you will unlock this achievement.
The Purpose of a Thief
Obtained a Treasure Demon.
Rare shadows spawning in Palaces or wandering around in Mementos. Will be defined while playing in game.
Attack one with its weakness and befriend them through "Hold Up!". You need an empty Persona slot in your party.
Technician
Triggered a Technical.
To trigger a Technical, you will need to apply an elemental ailment to an enemy and then hit it with the right attack combo. The right combinations for a technical are:

Let's Blow It Up
Defeated enemies via Disaster Shadow explosion.
Disaster Shadowss are found in Palaces and Mementos. These shadows look like their normal variant but with a purple effect.
They will expload when defeated, dealing damage to the shadows on their side. Defeat one and make sure it deals enough damage to take the other enemies down with it.
It's Showtime!
Performed a Showtime attack.
Special move unlocked into the third palace. You will have to use it outside of the tutorial. Showtime Attack Popup is completly random,
but mostly triggered when enemies are low. You will find them sooner then you think.
I am Thou...
Obtained a Persona through negotiation.
Knock down all enemies in battle to trigger a "Hold Up!", there you can request for Money, Items or recruitment of the Persona.
When selecting the Recruit option, answer the questions matching with its personality.
Jose's Favorite Customer
Traded in flowers while in Mementos.
Flowers are a collectible in Mementos. When entering a floor in Mementos, you will see a cutscene where Jose is driving into the area.
Find him and trade an item, this will unlock the achievement.
The Phantom Philatelist
Traded in stamps while in Mementos.
Refer to the The Search for Power Achievement below.
The Search for Power
Completely changed the cognition of Mementos.
Collectible found in Mementos. One stamp can be found at the end of each platform track and others scattered randomly on each floor. On the Mementos Overview you can keep track of the remaining stamps for each section. Can be traded with Jose for changes of XP, Money or Item multiplier. You will have to max out one out of the three categories.

The Deviated Cognition
Encountered a deviation in Mementos.
Deviation are random events where the Mementos floor is completly dark. This happens compeltely random but isn't that rare.
Talent Thief
Obtained a Skill Accessory from a Palace ruler.
Missable - Every Palace in the game has 3 Will Seeds, which are found before defeating the ruler of the palace. After finding the third one it will merge into a skull item.
Bring in to Jose in Mementos and obtain from him an accessoire.
Unsurpassed Rebel
Conquered the Reaper.
Missable - The Reaper is an powerful enemy encountered after beeing to long in a Mementos floor or simply waiting for 2 minutes.
Be prepared to have a higher level to have a fair chance against him.
A Deadly Debut
Performed a Guillotine execution.
First unlocked method to fuse two Personas into a new one. Use it once for unlocking the achievement.
Efficent Executioner
Performed a Group Guillotine execution.
This type of fusion uses 3 or more Personas to get an powerful new Persona. Requires Strength Confidant Rank 1, bring a Jack Frost with Mafufu to the Twin Wardens.
A Grand Experiment
Performed an Electirc Chair execution.
This execution method allows your to transform any of your Persona's into useful items or skill cards. Will be unlocked in August.
Intensive Training
Used Incense on a Persona in Lockdown.
Will be unlocked after unlocking Strength Confidant Rank 3. After use an incense on your Persona and unlock the achievement.
Success Built on Sacrifice
Performed a Gallows execution.
Unlocked after securing a route of the fourth Palace. This execution will boost one Persona by sacrificing another.
Accident-Prone
Performed an execution during a Fusion Alarm.
A fusion alarm is triggered after a random amount of battles, and can be increased through a Celestial Reading. A animation of a red door will play, followed with the message: "I'm getting an uneasy feeling...". Head to the Velvet Room and perform an execution.
Professional Modification
Customized a gun.
Requires Hanged Man Confidant Rank 1, which requires the social stat Guts Rank 4. After that you can customize guns at the air soft store in Shibuya.
Leblanc Buffer
Cleaned up in Leblanc.
Missable - Interact with the closet on the first floor in Leblanc, next to the stairs.
Punch That Clock!
Worked a part-time job.
Missable - Easiest way is to read the pamphlets and call the job, you want to start. Travel to the job site and work a shift.
Batter Up!
Scored a hit at the batting cages.
Can be found in Yongenjaya, around the corner from Leblanc. During the day only five chance and during night unlimited pitches. You will have to score one hit.
Getting the Vapors
Couldn't take the heat in the bathhouse.
Found right in front of Leblanc and only during rainy days to unlock the achievement. Outcome is random so you will need to do it multiple times.
Easy Money
Won the lottery.
You need to purchase a ticket at the Shibuya train station, right next to the Sun Confidant. Buy a daily or weekly ticket. Winning is random, so come back after the duration and buy a new one if you didn't win.
Angler's Debut
Passed time at the fishing pond.
Accept the invitation from Ryuji on July 6th or read the book "Fishpond Spotter" available at the Shibuya bookstore. Travel to the pond and play the fish mini game till catching a fish.
Dartslinger
Played a game of darts.
The bar can be found in Kichijoji. Unlocked after playing the mini game.
A Hustler's Journey
Played a game of billiards.
The bar can be found in Kichijoji. Unlocked after playing the mini game.
A Serene Experience
Visited a temple.
Found in Kichijoji. Visit during the day by speaking with the old man standing outside. After interacting will trigger a cutscene.
Trash Into Treasure
Sold old clothing at Furugi no Neuchi.
Located in Kichijoji. Used to get rid of old or sooty gear.
A Night in Kichijoji
Attended the jazz club.
Requires Justice Confidant Rank 4. Go at night to the jazz club.
Going Against the Crane
Scored a prize in the crane game.
Crane machine is found in the arcade in Akihabara. You can play every day for a chance to win. Unlock the achievement after you won.
Master of Akihabara
Ordered from the special menu at the maid café.
You will have to go to the maid café in Akihabara for 4 times and order the most expensive item to access the "special menu"
Tokyo Tourist
Went to a hangout spot with someone.
Hangout spots can be unlocked through reading about them in magatines, through scripted story event or leveling up confidant. After unlocking them invite an confidant to join you and hangout there.
True Confidence
Maxed out one of your Confidants.
A confidant can be maxed out when their Rank reaches 10.
  • Spend significant time with them and have a number of social interactions with them
  • Use available hangouts
  • Choices in dialogues matter
  • Ann and Ryuji are the easiest and quickest to max.
My Closest Partner
Entered a special relationship.
Missable
  • In order to enter a "special relationship" you will need to first reach Rank 9 with one of the girl confidants. Most of them can be romanced, however some have special pre-requisites before you can begin a special relationship with them. At Rank 9, there will be a prompt telling you that you need to "choose your words wisely". This is how you know that the next line will have you select whether or not to enter a relationship with that confidant. The line that begins the romance is fairly telling.

  • The following confidants have no pre-requisites and can be romanced at Rank 9:
    • Ann Takamaki (Lovers Arcana)
    • Makoto Nijima (Priestess Arcana)
    • Haru Okumura (Empress Arcana)
    • Futaba Sakura (Hermit Arcana)
    • Sadayo Kawakami (Temperance Arcana)
    • Hifumi Togo (Star Arcana)

Evolved a party member's Persona.
Missable - Enter the third semester while going for the True Ending. Will be unlocked automatically.
A Most Studious Disguise
Scored the highest on your exams.
Missable
  • You will need to answer every question correctly for all exams on the following dates:
    • May 11-14
    • July 13-16
    • October 17-20
    • December 20-23
  • You will also need to max out your Knowledge social stat by answering correctly in class, reading books and studying in order to pass these exams with the highest score.
Pure Perfection
Maxed out all social stats.
Missable
  • Most social activities throughout the game will give you points towards certain social stats. Reading books, playing games, watching movies as well as interacting with some confidants will all contribute towards this achievement. For it to unlock, you will need to attain the Rank 5 in the following: Knowledge, Guts, Charm, Kindness, and Proficiency.
  • In case you do not manage to rank them all up to maximum by the end of the game, you can carry over the progress in NG+.
