In QL you can change a lot about the gameplay by tuning existing cvars, but there is no real modding ability. That would be required to add a true off-hand-hook similar to q2 lithium
just release the thumb button (-hook triggers) and press mouse1. it's an instant switch from hook to firing a rail.
only thing you cannot do is fire while you are camping on the ceiling with the hook
or 45.79.100.154:27961 (US west coast) for servers which run this mode.
If you have extraQL installed and the InstaBounce userscript enabled, it will automatically bind your mouse buttons when you join an insta bounce match - and restore your old config when you leave.
The workshop item adds a "ibounce.cfg" script which you can /execute.
That will bind mouse2 and mouse5 to aliases which switch weapon + fire when you press the button, and switch back to rail and stop fire when you release the button.
When you start a match with any of the ib_* factories (e.g. /map campgrounds ib_ffa), it will reduce the weapon switch time as much as possible so the switch feels almost instant.
