Pretty much all of the achievements are self-explanatory. This guide will outline some tips for maximizing efficiency to attaining 100% completion without wasting too much time or energy.

Finish the story on "maniac" difficulty
Save, Save, Save
The game has a great autosave system with fair and balanced checkpoints that will easily keep you satisfied as you play through the game. That said, you should create a new manual save at the start of each new level. This way, if you need to mop up any extraneous achievements at the end of the line, you will have a nice little level-select system which you created for yourself.
Sidequests
This probably doesn’t need to be said, but be sure to not skip any sidequests. There are a handful of achievements tied to completing them. You will want to explore every corner on your initial playthrough anyway to get the collectibles and collect money for upgrades.
Lord of War
In order to get Lord of War: you are going to have to play through the game nearly twice. If you follow these guidelines, you will minimize the total amount of time required to complete this and won't have to complete the second playthrough entirely.

  • Don’t upgrade the pistol. Finding all of the collectibles for A shimmer in the darkness will unlock the Golden Gun, and it will automatically max out your pistol upgrades.

  • Save your money early on and do NOT spend it on upgrading the shotgun, rifle or grenade launcher.

  • When you attain the late game weapons, only then spend your money on upgrading those weapons (flamer, rocket launcher, laser rifle).

  • If you have extra money before the final boss, spend it on upgrades working backwards from when you attained the weapon.

  • After you beat the game, start a new game. Your upgrades and collectibles will carry over. Plow through the levels, grabbing money. You should be able to finish upgrading the shotgun, rifle and grenade launcher a little more than halfway through the game.

  • By doing this you won’t be forced to keep playing to re-attain the late game weapons, since they will already be upgraded from your initial playthrough.
The Looter
Nothing in common but the enemy and Can't trust anyone are mutually exclusive and are linked to a decision in chapter 2. Since you need to play Level 2 twice anyway to work towards Lord of War, just be sure to choose a different option each time.
Wrecking Ball and SKiller
It is probably best to aim for both of these achievements on your second playthrough of Level 2, while working towards Lord of War. You will have the Golden Gun already and fully understand the mechanics of the game by then. Just beat the boss without getting hit and only using the pistol. Easy.
Secrets Areas and Collectibles
A Key moment and Secret Cave are unlocked by finding secret areas in the village map (Level 5). It must be Art can be unlocked in the bug cave level (Level 5.1). They are pretty easy to find while you diligently search for money for upgrades.

The collectibles needed for One more layer between them and me and A shimmer in the darkness are scattered throughout the game. They are also pretty easy to find while searching for money.

"Maniac" Mode
For This madness had to stop, the game only checks if you’re set to “maniac” difficulty at the exact moment the ending scene begins, so you don’t need to play the entire game on that difficulty. However, you will first need to beat the game to unlock “maniac” difficulty. You can create a savegame during the final boss encounter and then reload that save after initially beating the game. Pause the game, switch to “maniac” mode and quickly finish the game again to unlock this achievement. You can even save the game immediately after the boss dies, as there is a 5 second window before the ending scene begins which you can use to pause the game and change the difficulty.

Yes, this does undermine the intentions of the developers and could be considered “cheating.” However, it only involves manipulating the achievement check system and doesn’t involve editing game files or using “hacks” in any way. You will have to decide for yourself if you are comfortable with this or not. If not, you are free to play the whole game on “maniac” mode.
Survival Mode and Score Multipliers
You will want to go for the score multiplier achievements (Ultrakill! and MO-MO-MONSTERKILL!) while playing Survival Mode after maxing out all the weapon upgrades in story mode. The upgrades carry over to this mode and will be quite helpful. You will need to get to 250,000 points on any map to unlock Survivor, Club of 100,000, and This is Madness!. It may take some practice, but it’s doable. I did these on the map Crimsonland.

A key moment - Chapter 5, on a table next to a firepit/big red dot in the same house as the secret/locked door.

This madness had to stop - autosave works for part 3 of final boss, after credits/epilogue, continue game, options, maniac.
