Read first part of the letter. You can find it in room 101 after accident with the cat.


Read second part of the letter. You can find it on the second floor. When passing through corridor with windows you can hear faint whispering, stop walking and listen. After being exposed to somewhat gruesome imagery and poetry, you can find the letter part in this corridor.

Call Doctor Z "full of crap", but don't deny his request. Later during the "procedure" call him "full of crap" again. As TetraSunrock pointed out, achievement does not trigger when during second conversation you go with "I'm beginning to wish I've never met you". So the safest way to get it is following:
1st conversation - choose "you're full of ♥♥♥♥" and then "Fine. I'll go to the cellar"
2nd conversation:
1st dialogue: choose "Shut up and do it already" or "You can't just plant a dead brain inside her head!"
2nd dialogue: choose "Now I'm sure. You really ARE mad"
3rd dialogue: choose "You're so full of crap I haven't been really listening..."

Playing as Agnes, turn the radio on, when you're locked in the room.


Playing as Agnes, break the statue and take the penis.


Playing as Agnes, go into the maze, take the doll and go outside the maze (note: don't talk with Ivy yet). Slendy will be out there in the woods.

Playing as Agnes, convince Ivy to go with you. It's pretty easy - just be stubborn about it.


Playing as Agnes, persuade The Queen of Maggots to give you her real name. Don't worry, you won't have to eat the soup.


Playing as Joe, speak with the dead, when you're in the coffin.


Blow out the candle in Queen of The Maggot's home.


After you get back to the hotel, you'll have to push the piano away, 'cause it's blocking entrance to the lift. You can play the piano afterwards (play all the way through), which will trigger the achievement.

Visit all Floors.


Tell Harrison to ♥♥♥♥ off. He deserves it.


If you picked up the second part of the letter, you can find the third part in secret room, where Doctor Z met his gruesome end.


Examine panties left by manageress.


When you get the axe, kill the manageress.


When you get the axe, kill Agnes.


In case you choose to kill Agnes, after doing the deed, go to Manageress office. You'll find Agnes' head. You can't interact with it and sometimes the achievement doesn't trigger immidiately.

You'll get this automatically by dealing with all four Sophies.


Achieved by doing everything there is in house at Helen Road.


When Doctor Z pays you last visit, say that nothing is impossible and you'll get motivational speech.

When Playing as Susan construct both possible weapons - The Mannequin Axe and The Cat Scythe. Detailed description: As Susan go to the open apartment, take a rope from the bedroom and go through the hole in the floor in previous room. Take broomstick. Take the mannequin hand in the corridor, take blade and rag from the bathroom, go to the lowest level and use mannequin hand on the manhole. You'll get the key to cabinet at Joe's apartment. Go there and and take the duct tape from the cabinet. Go out of the apartment and use rag on the glass under axe case. Go back to lower level and go to the room with crafting table. Open the oven and take cat skull. Now just put everything you have in your inventory in the crafting table window and Susan will do the rest for you.

Achieve Golden Ending or Downfall without tendency hints.


Turn off the autosave in menu -> options before starting a new game and beat the game without turning it back on.


In the menu go to Extras and then to Lucky Dip. Repeat until you get achievement.
