How to get the "The Full Truth" Achievement

Here is a basic guide to get the "The Full Truth", both in text and video format. My next guide will be the All Ticket Locations

The first 4 questions can be earned during Act I

Once asked, exit to the main menu and restart the Act 4 times for the first 4 questions

Who was I?
Who killed me?
When did I die?
Where did I die?

Once done you'll need to replay Act III in order to be able to ask "How did I get here?". Once done the achievement should pop. If not then try again and make sure to hit back before leaving

IMPORTANT NOTES

There are 3 locations to get these. Act I during the Menu Screen, Act II during the World Map in the Cave of Truth or Act III. Questions 1-4 can be obtained at either of these locations but "How did I get here?" can only be asked during Act III

It isn't possible to ask all questions in a single game

@JuicyMan I no longer have any clue how this guide works anymore. I've gotten too many conflicting info from people who say it works and those that say it doesn't. I also have replayed this game after perfecting it. Steam user @walter.comunello has made their own guide that is a lot more up-to-date and goes into more depth than my video.

I have what I think is a definitive answer on how to consistently get this achievement. You definitely need to have seen all three visions for "How did I get here?" to show up in Act III, but beyond that it seems that you also need to see them in order.

For me, the "How did I get here?" option did not show up after 10 or so re-plays of Act I. I also was not able to find it after 5 re-plays of Act III. Only after re-watching the visions from Act I, then Act II, then Act III in that order did I see the "How did I get here?" option show up in Act III. Others have shared screenshots of proof that this text option can also be seen in Act I, but my assumption is that this is only possible after watching the three visions in order. That is what worked for me, anyway.

There may be other ways to get this achievement, but nothing worked until I tried this.

I think you need to get all three visions from each of 3 locations to showing off the last question.

I also got it in Act I, after NOT getting it in Act III when the requirements were there

I got "How did I get here?" in Act I after asking 3 different questions during the playthrough (in Act I, II and III). The question appeared in Act III, I just didn't pick it this time. Since I didn't wanted to replay parts of the game, I just went into Act I after finishing the game and gave "Baphomet" his Password twice to get the remaining answers to the 5 questions. So I guess that also works.
