Story
The following achievements are story related and thus unmissable. For some achievements, certain actions must be completed to unlock them:

On My Own
Complete the Prologue.

Victory is Mine
Complete Chapter 2 - Defeat Mulahey at the Nashkel Mines, then take the letters off of his corpse after the fight.

Bandits Banished
Complete Chapter 3 - Defeat Tazok at the Bandit Camp, then acquire some letters from the chest in his tent.

Water, Water Everywhere
Complete Chapter 4 - Defeat Daveron in the Cloakwood Mines.

On My Own... Again
Complete Chapter 6.

Hero of Baldur's Gate
Complete the game.
Side Quests - Part 1
The following achievements can be acquired at any time upon beginning Chapter 1:

Foreshadowing?
Complete the quest "A Book for Firebeard".

Mercenary of the Month / Worst Bodyguard Ever
Complete the quest "The Silke Dilemma" by doing one of the following:
  • Mercenary of the Month - Agree to kill the men, then talk to Silke once the deed is done.
  • Worst Bodyguard Ever - Refuse to kill the men, and kill Silke when she turns hostile.
Both achievements can be acquired in a single playthrough by saving before completing one path, reloading, and then playing out the other path.

Only Mostly Dead
Complete the quest "Drienne's Cat".

The Written Word
Complete the quest "Tomes of Magic".

Scent of a Kobold
Complete the quest "The Firewine Ruins" - The quest can be acquired either in Gullykin Village, or from entering and exploring the dungeon at Firewine Ruins.

Fishermen's Friend / Fishermen's Foe
Complete the quest "The Fishermen and the Priestess" by doing one of the following:
  • Fishermen's Friend - Attack Tenya to near death condition, refuse her request and kill her, then return to Sonner.
  • Fishermen's Foe - Attack Tenya to near death condition, agree to her request and confront Sonner, then return the bowl he gives to Tenya.
Both achievements can be acquired in a single playthrough by saving before completing one path, reloading, and then playing out the other path.

The following achievements cannot be acquired at any time, until you reach Nashkel and begin Chapter 2:

Philistine
Complete the quest "Prism the Sculptor".

Witch Slayer / Witch Savior
Both the quest "Edwin and Dynaheir" and "Rescuing Dynaheir", given by Edwin and Minsc respectively, concern the companion Dynaheir, found within Gnoll Stronghold. Although Edwin's quest must be completed for Witch Slayer, Minsc's does not need to be acquired to unlock Witch Savior - simply finding and recruiting to the party is sufficient enough. However, both achievements can be acquired in a single playthrough by saving before completing one path, reloading, and then playing out the other path.

Judge, Jury, Executioner / Second Chance
Complete the quest "The Tale of Captain Barge" by doing one of the following:
  • Judge, Jury, Executioner - Kill Barge by either telling Laryssa you're here to kill him, fail Barge's riddle, or solving the riddle but telling him you are here to kill him.
  • Second Chance - Save Barge by agreeing to spare him. You must speak to Nalin in Nashkel first, agree to spare him for Laryssa, solve Barge's riddle and then bring him back to Nalin.
Both achievements can be acquired in a single playthrough by saving before completing one path, reloading, and then playing out the other path.

Sobering News
Complete the side quest "Joseph's Wife".
Side Quests - Part 2
The following achievement cannot be acquired until you begin Chapter 4 and have access to the Cloakwood:

Forest Friend / Noble Assistant
Complete the quest "Aldeth Sashenstar" by doing one of the following:
  • Forest Friend - Side with Seniyad, then kill Aldeth and his friends.
  • Noble Assistant - Side with Aldeth, then kell Seniyad and his druids.
Both achievements can be acquired in a single playthrough by saving before completing one path, reloading, and then playing out the other path.

The following achievements cannot be acquired until you begin Chapter 5 and have access to Baldur's Gate:

Master Thief
Complete the quest "Larceny at the Hall of Wonders".

Reunion
Complete the side quest "Tremain's Son".

The following achievements can be acquired at any time from Chapter 1, by entering Ulgoth's Beard:

The Legend of Durlag's Tower
Complete the quest "Durlag's Tower" - This requires defeating the Demon Knight within the final level of the dungeon at Durlag's Tower.

An Ancient Evil
Complete the quest "Recovering "Soultaker"" - This quest will unlock after acquiring Soultaker from the Demon Knight's corpse and returning with it to Ulgoth's Beard. The fight with Aec'Letec, that you must undertake, is difficult to complete but is easiest done on Story Mode.
Difficulty & Character Statistics
The following achievements are acquired either by completing the game on certain difficulties or under certain conditions. It is not possible to acquire all in the same playthrough - it will require at least two playthroughs to unlock these:

Friend of Volo
Complete the game on Story Mode difficulty, without changing it at any point during the playthrough.

Lord of Murder
Complete the game on Legacy of Bhaal difficulty.

Flying Solo
Complete the game with only your own character - do not recruit any companions during the playthrough.

Leave No Friend Behind
Ensure that no companions you recruit die throughout an entire playthrough. This can be easily achieved by completing the playthrough on Story Mode difficulty. Be careful on other difficulties, especially with the fight against Aec'Letec and Sarevok.

Sacrifices Must Be Made
Complete the game's final fight with only the main character being alive. Any and all companions for the fight must be dead when Sarevok is killed, as the achievement will not unlock if at least one is still alive. The achievement is not possible when playing on Story Mode.

The following achievements are acquired by getting any character - whether created by the player, or a companion (depending on the achiement) - to certain attribute/skill levels:

Juggernaut
Increase a character to 150HP. This is best achieved with either The Black Pits or Siege of Dragonspear, by creating a half-orc Fighter character for either game mode, and setting their Consitution to 19. At Lvl 7 they will have about 110HP (particularly when beginning Dragonspear), so will have a considerable amount when reaching the cap of Lvl10. Don't forget to make use of Potion of Heroism (+10% HP) and Potion of Power (+20% HP) to help edge the HP to the target level.

Godlike
Increase any attribute score of a character to 25. This can be easily done by using multiple Potion of Mind Focusing, as the effects are stacked. It's also possible to use the potions bought from a merchant at the Nashkel Carnival, as this will increase one attribute to 25 at the cost of another.

Untouchable
Have a character achieve an Armour Class of -15. This is best achieved using the Fighter class with characters used for Black Pits. To achieve this, ensure the character has a Dexterity score of at least 18 to begin with, and then acquire and wear the following items when they become available:
  • The Dusty Rose Ioun Stone Helm (-1 AC)
  • Pellans Shield +2 (-3 AC)
  • The Guards ring +2 (-2 AC)
  • Improved Cloak of Protection +2 (-2 AC)
  • Boots of Phasing (-2 AC)

To ensure the target value is reached, players may also use Potion of Defense and Potion of Agility, and also can use multiple Potion of Mind Focusing, the latter able to raise Dexterity to 25.

Loremaster
Have a character with a Lore skill of 100. This is best achieved with a Bard character created for Siege of Dragonspear. They earn 10 points to the skill for each level they achieve, and an additional bonus for having high Intelligence and Wisdom, so maxing out this scores during creation is a must! To increase the score further, using Potion of Wisdom and a few Potion of Mind Focusing can also help if needed.

Grand Master
Have a character achieve Grandmaster with any weapon. This is best achieved with a Fighter character created for Black Pits or Siege of Dragonspear. When they level up, focus on one weapon skill to improve to unlock the achievement.

Hero / Villain
Of the two achievements, Villain is the easiest achieved, as you only need to kill people and reach the Despised reputation level. For Hero, you need to focus on committing to good actions and resolving quests in a good nature, in order to raise your character's reputation level to "Heroic". Both can be achieved in a single playthrough by saving the game and focusing on one achievement, then reloading to unlock the other.

Murder Incarnate
Kill 1,000 creatures. The game counts kills made by ambushes when travelling and resting outside settlements, so the best place to rack up the kills is by travelling and resting around the Gnoll Stronghold.
Miscellaneous - Part 1
The following achievements can be obtained at any time in the main game, or during Black Pits or Siege of Dragonspear:

Oathbreaker
This can only be done with a character whose class is either Paladin or Ranger. To unlock the achievement, cause that character's reputation level to drop to around 5 or lower, to change them into a Fallen Paladin/Ranger. Can be worked on alongside that for the achievement Villain.

Best Day Ever
Have a character under the effects of of Bless, Luck and Chant at the same time. Both Bless and Chant are AOE priest spells. However, Luck is a mage spell which must be directly cast onto the character.

Worst Day Ever
Have a character under the effects of Fatigue, Poison and Disease. Poison is a priest spell, and Contagion is a mage spell that inflicts Disease, both of which must be directly cast onto the character. Fatigue can be achieved by the character being active for long hours without a rest. Be advised that Story Mode can thwart attempts to unlock this achievement, and the character's saving throw can interfere with acquiring Poison and Disease.

The following achievements can only be obtained during the Prologue:

Back to Basics
Find and talk to all the tutors in Candlekeep during the Prologue. There are fourteen to find - twelve around the main grounds, and two within the inn - and all are easily recognizable as they wear green robes and bear the name "Tutor".

Completionist
Complete the following side quests during the Prologue:
  • Dreppin's Cow
  • Phyldia's Book
  • Reever's Storehouse
  • Hull's Sword
  • An Errand For Fuller
  • Firebead's Scroll

Safe For Now
Find and kill Carbos and Shank during the Prologue. Carbos is located in the Bunkhouse and Shank in the Priest's Quarter.

The following achievements can be unlocked at any time after beginning Chapter 1:

Friendly Faces at the Friendly Arm
Travel to the Friendly Arm Inn, and then find and talk to Khalid and Jaheria; you do not need to recruit them.

He Started It / Use Your Words
When visiting Feldepost's inn in Beregost for the first time, you will automatically enter a conversation with an NPC called Marl. Do one of the following:
  • He Started It - Simply threaten him, and then kill him.
  • Use Your Words Choose conversation options 1,1,3,3,1.
Both achievements can be acquired in a single playthrough by saving before completing one path, reloading, and then playing out the other path.

Entertained
Recruit Baeloth to your party. He can be found in Larswood, and is recruitable after a scripted scene involving him. Be mindful that he is an Evil character when you try to recruit him.

This Belongs in a Museum / Gallors Plot / Faerûnian Idol
Visit the wilderness area south-east of the lighthouse along the coast, and explore the map until you find a dig site. When you do find it, do one of the following:
  • This Belongs in a Museum: Agree to help Charlston with his request, refuse Gallor's request, then venture into the tomb. Deal with the fight that soon happens in inside, then talk to Charlston to unlock the achievement.
  • Gallor's Plot: Agree to Gallor's request after agreeing to help Charlston. Venture into the tomb and then kill Charlston inside. Take the idol from the sarcophagus and bring it to Gallor to complete his task and unlock the achievement
Both achievements can be acquired in a single playthrough by saving before completing one path, reloading, and then playing out the other path.
For the third achievement, Faerûnian Idol, simply take the idol from the sarcophagus after doing This Belongs in a Museum, leave the tomb, and then defeat the creature that attacks you.

Unpetrified
Head to the wilderness area north of Gullykin, and use the priest spell "Stone to Flesh" (or a scroll) on a statue around the north-west portion of the map spell. The statue is easy to identify as it will be the only one to be interacted with. Be mindful of basillisks in the area.

Save the Cow!
Head to a wilderness area south of Ulcaster, and search the map until you find an NPC called Hurik. When you do, agree to help protect the cow near to him. Defeat the enemies that appear, and the achievement will unlock if the cow is still alive, after talking to Hurik after the fight.

The following achievements can be unlocked at any time after beginning Chapter 2:

A Work of Art
Find Prism near the Nashkel Mines and agree to help him - you do not need to be on the quest "Prism the Sculptor" to complete the achievement. After agreeing to his request, refuse to let the Greycloaks, that soon arrive, to harm him and defeat them, The achievement unlocks after Prism speaks to you one more time when the fight is over.

All the Options
Have Neera, Dorn and Rasaad in the party at the same time. Rasaad and Neera are the easiest to recruit - the latter before Chapter 2 - but Dorn is trickier. Apart from being an Evil character, he can only be recruited during a special encounter when travelling to the Nashkel Mines. To activate this, you must find and talk to him at the Friendly Arm Inn.

The Stone Cleric
Use the priest spell "Stone to Flesh" upon the statue of Branwen at the Carnival. If you don't have the spell on any priest character, you can use a scroll - Zeke offers one nearby, but is cheaper to get one from a temple.

Miscellanous - Part 2
The following achievement can only be unlocked after reaching Chapter 5:

Damsel Out of Distress
Rescue Abela in Baldur's Gate. This can either be from rescuing her from Ragefast during the quest "The Captive Nymph", or from Ramazith if you acquired the quest from him and send Abela to him; though you will have to traverse his tower and kill him to do this.

With Ragefast, you can either kill him, or convince him during the automatic conversation upon entering his home. The latter option, however, requires your main character to have a Wisdom score of 7+ in order to use the following conversation choices - 2,2,1 - to be successful and decide on Abela's fate.

The following achievements are only available in Chapter 7. Each can be achieved in the same playthrough, as long as you save before visiting the Ducal Palace:

Lover's Sacrifice
Kill Tamoko in the Undercity.

Lover's Redemption
This achievement will require a character with a high Charisma score - a paladin, druid or bard are good choices, along with them wearing any items that boost Charisma. To unlock the achievement, meet Tamoko within Baldur's Gate and agree to her request. Follow her to the Flaming Fist Barracks and help her kill Cythandria. When you encounter Tamoko in the Undercity, you must use the conversation options 1,3,2. If done right, Tamoko will walk away, unlocking the achievement.

Though it's a known answer, I hope to spare someone's time searching it. Meeting Imoen makes not able to get the achievement "Flying Solo" at the end. You should prevent meeting her by not stepping in the area she's in. If your party consists of 2 characters at any point of the game, you won't get the achievement. You can complete Garrick's quest without recruiting him. Since a lot of others achievements are from companion's quests, better try your chances at the 2nd or 3rd playthrough with story mode on.

@Zolvolt I am a it confused about those achievements. The guide says you can get them in Siege DLC. Is it like this?
Zolvolt 22 Oct, 2023 @ 10:08pm
@Seagoblin if it helps the only playthrough before Siege that takes a long time is your initial one. You can grab most achievements on that playthrough following a guide. I used:

https://gamefaqs.gamespot.com/ps4/258143-baldurs-gate-enhanced-edition/faqs/78542/appendix-lord-of-murder

beware they sometimes don't know compass directions. I also used this video

for the easiest and fastest legacy of baal run you'll ever find.

im so tempted to give this game a 2nd shot. but dang the road map for 100%ing all the achievements is like 120 hours minimal almost. which kills my soul lol. have had the game installed for 2 years now and putting it off cuz i just am not ready for the commitment to play it with a 100% road map that takes that long lol

I'm working out which achievements are tied to your main character only. So far:

Loremaster: have 100 lore skill (level 10 bard pre-expansion)
Untouchable: Get -15 AC
Godlike: Have 25 in one stat
Worst Day Ever: 3 specific debuffs
Best Day Ever: 3 specific buffs
Oathbreaker: Fallen Ranger/Paladin
Juggernaut: Get 150 Hitpoints (btw just save up all your potions of power, fortitude, and heroism and use them all at max level. Draw upon Holy might also if running a Good Bhaalspawn character)

To be determined:
Grandmaster?
Zolvolt 3 Oct, 2023 @ 11:17pm

Is achievement Flying Solo still possible after Imoen forcefully inserts herself into the group after leaving the Candlekeep?
