So few achievements make Atari Vault an easily achieved perfect game.

Atari Vault has only twelve Steam achievements.
The description was ironic because these achievements take a lot of repitition and practice. It requires you to be good at a wide variety of Atari games and that doesn't include my favorite. (Combat)
Wanderer
Play a game from every category in the List View.
Easiest Achievement, takes no skill just time.
Exterminator
Take out an entire Millipede with DDT in the arcade version.
A DDT domb makes a cloud-stlye explosion so you must line up the shot and wait for the Millipede to cross paths and shoot the DDT bomb right before it gets there.
We Can Rebuild
Earn a bonus city in arcade Missile Command after losing all your cities.
At the end of each level you get bonus points for any cities you have left or unused missiles. Between levels missile batteries are rebuilt and replenished; destroyed cities are rebuilt only at set point levels (usually every 10,000 or 12,000 points).
You must time a city to be rebuilt on the exact same level when you lose all of your own and Achievement Get!
Sniper
Take out the Swirl with your Zorlon Cannon in Yars' Revenge.
This achievement is also pretty self explanitory. Wait until the Qotile turns into the Swirl and use your Zorlon Cannon to destroy it.
Lockout
Win a game of Pong against the AI without losing a point to it.
Lockout in my opinion is one of the easiest achievements. I found Pong harder with some control imputs but when you have your figured out its a piece of cake.
Sharp Shooter
Take out 10 fleas in a row in arcade Centipede, without letting one reach the ground.
A flea appears when there less than 5 mushrooms in the active zone that you can fly. Their job is to keep 5 mushrooms there at all time so keep destroying them to keep summoning more fleas. And as the achievement states, don't let any of them reach the ground.
Brick Slayer
Clear a full screen in arcade Super Breakout while playing cavity or double. These game modes are 3 to choose from and the achievement is only obtained when playing in Double or Cavity.
Choose either gamemode you're comfortable with and clear a full screen.
Double: The player has control over two bats and one time. You must keep both balls in play to keep your lives.
Cavity: You only have one bat and one ball. However, two other balls are trapped on the other side of the wall. The player can release these extra balls to have up to three going at once.
Hare Razing
Get the top score of 99 in Air-Sea Battle's Shooting Gallery.
This achievement is obtained by getting a score of 99 in a shooting gallery game (13-15). This achievement is tough but can be obtained with consistent play.
Stealthy
Win Level 2 without slaying a single dragon in Adventure.
This achievement can be accomplished simply by dancing around the dragons in level 2. This achievement isn't as hard as it sounds. The dragons are easily predictable and you can look up a map of where they spawn and how to avoid them.
UFO Hunter
Kill 10 UFOs in a single level of arcade Asteroids without dying.
This achievement is a tough one for me simply because I don't like Asteroids very much. The UFOs come into play every once in awhile and there are two of them. A smaller one and a larger one. To get 10 in one level then shoot the asteroids down to one small one and then just avoid it until 10 eventually show up.
Space Jockey
Earn 800 points in Lunar Lander's COMMAND MISSION.
Very difficult achievement, Lunar Lander's COMMAND MISSION is tedious and annoying. 800 points seems like an eternity on a boring game however it is accomplishable.
We Don't Need No Fortification
Win with no wall left in arcade Warlords.
This achievement has a bit of luck in reality. You have to let down your wall and defend the open spaces which can be difficult if on opposite ends. The best way to that is to slowly only allow the ball to damage one side and work your way to the other. It takes a lot of skill and can be easily messed up with the slightest mistake. Keep at it and it will eventually happen.

SUPER BREAKOUT ACHIEVEMENT FIX:
It seems as if you must beat a screen while in the applicable modes while the game is in color. For some reason after a certain amount of matches the game will go monochrome and weird tiling glitches will be seen on the right side of the screen. Here's how to ensure you get color mode when starting a game:

1. Keep trying to get the achievement in your preferred mode until the game turns monochrome.
2. Pause and choose "Main Menu".
3. From Options, switch your game mode to any other than the current mode you have selected, and resume your game. If done correctly, the screen should be mostly black.
4. Press "Reset" on the pause menu for the new mode to initiate in color.
5. Go back out and change your Options back to the mode you're using to get the achievement.
6. Repeat step 4 once having gone back into the game, and you should now be able to keep trying until either you need to repeat this process OR you get the achievement, in which case, congrats!

I clear the whole screen in Super Breakout on Double and it didn't give me the achievement...

Not much of a guide. you don't give much information other than the actual achievment description plus how hard you think it is.

Another easily achieved perfect game is turok, with all achievements possible in one run(most are part of the actual story)
