With an USB/Xbox/PS Controller press:
(The cursor must be over the "Start" buttom on the title screen.)

PS/USB Controller:
Triangle, Square, O, Triangle, Square, O, X

XBOX Controller:
Y, X, B, Y, X, B, A

Keyboard(Thanks to BlueBear):
I, J, L, I, J, L, K

You'll hear Axel's voice if it worked.

You either enter a code (For low level play-through) or unlock it through game completion (High level).

Am I missing something? Axel mode was available when I beat the game without using any codes

@kekin It's 4 chapters but it can take a long while depending on how long you take to grind. It's also home to a really good grinding method involving thieves and poison and sleep.
