Locate the map in your maps/workshop folder (\Steam\SteamApps\common\Portal 2\portal2\maps\workshop), rename the .bsp file to some easy name like "fim_01.bsp" and move it into the maps folder. Then you just go into any map with your coop partner and type "changelevel fim_01.bsp" in the console.
If you can not find the map files, i uploaded them {ENLACE ELIMINADO}here. Just unpack and put them into your maps folder. (\Steam\SteamApps\common\Portal 2\portal2\maps)
