Very simple, go here and download the +18 patch, then copy the folder "game" to your install directory and launch the game.

Simple as that.

About the rest of the achievements
If you guys want i can make achievements for the choices and endings.

I'm in the same boat as the previous commentenor, followed the steps and did this, yet it still won't give me the achievement. Although, I have all others. -.-
