This guide will hopefully help you obtain all of the achievements in this game. Most of the achievements are self explanatory but a few may need some clarification.

Be aware that there are spoilers along the way.

This achievement will unlock when you grab the helmet in the airlock (the one that's bouncing around on the floor) and rotate it until you're looking at the face plate. At this point, you will be transported into the dark world.
The Cable Guy
(Re-wire the door)
Can't miss

Re-wire the door to the Study Library so it will manually open. The instructions on how to open the door is on the back of the welcome poster.
The Librarian
(Books are gateways to other realms)
Can't miss

Find the manual on the floor in the Study Library, rotate it to the back and you will be transported to the light world.
Data Driven Death
(Free the peg leg)
Can't miss

Use the anti gravity device in the Lab to move the machine off of the peg leg in the Lab. This can only be done in the light world.
Brown Collar Worker
(Look what the cat dragged in)
Can't miss

Find the collar in the vent (dark world), and rotate it until you're transported into the light world.
This one goes to 11
(Fix the generator issues)
Can't miss

When you are transported to the light world after you find the collar, in the vents, make your way to the Reactor and fix the generator.
Cargo Cultists
(Enter the cargo area)
Can't miss

After you fix the generator and power is restored to the Control room, use the magnetic lift to clear the obstruction from the entrance to the cargo hold.
Key Account Manager
(Find 3 keys)
Hard to miss

Light world only. The keys are used to open the door to the cave. One is in the Medical bay, another is Phillip Pohl's closet, and the last is on the tram in the cargo hold.

NOTE: It appears that you need to have all three keys in your inventory in order to obtain this achievement. If you insert one of the keys into the keyhole before finding all three, you may not get this achievement. Thanks goes to kazereal for pointing this out.
Vacuum Sucks
(Dry the wet floor)
Hard to miss

Dry the floor in the Hydroponics bay before entering the Lab.
The Greatest Fan
(Unconventional use of unconventional tools)
Hard to miss

Use the anti gravity device in the Lab to break the fan so you can enter the vents. This can only be done in the dark world.
The Wild Bunch
(Collect all ID cards)
Hard to miss

Locate all of the ID badges scattered throughout the base.

It makes no difference if you're in the light or dark world, all of the medical reports can be found in the Medical bay.
Ready or Not
(Here I come)

Possible to miss
I obtained this achievement during the interview and I honestly don't remember how.

Fellow gamer, rogermorse said that he answered “B” for most of the questions, however, another fellow gamer, Skull said he answered “A”, but he answered immediately when the option appeared.

This makes me think that the questions are timed. So if you answer the questions immediately without reading them, you'll obtain this achievement, hence, Ready or Not.

UPDATE: [FWS] Telu, found out that it doesn't matter what you press as long as you press it quickly.
One way trip
(Watch the whole end scene and credits)
Possible to miss

When you enter the entity, you will see Karen Kowalski. All you have to do is watch until the achievement pops up.
The Other Cable Guy
(Re-wire the door to perform self testing)
Possible to miss

Re-wire the door to the Study Library so it will perform a diagnostic. The instructions on how to do this is on the back of the welcome poster.
The Paperboy
(Answer male in the interview and find all 6 magazines)
Possible to miss

Choose male in the interview and find all 6 magazines scattered throughout the base. Depending on your location, whether it's the light or dark world, the magazines will be in different places.

I didn't take any pictures of this achievement because this one is fairly easy to accomplish.
The Marathon Man
(Run 420 meters on a treadmill)
Possible to miss

Light world only. Find the treadmill in the Gym, start it, and run on it until the display reads 420 or more.
A Basket Case
(Make 12 points in Basketball)
Possible to miss

Light world only. All you have to do is make 12 baskets in the Gym.
The Papergirl
(Answer female in the interview and find all 6 magazines)
Possible to miss

Choose female in the interview and find all 6 magazines scattered throughout the base. Depending on your location, whether it's the light or dark world, the magazines will be in different places.

I didn't take any pictures of this achievement because this one is fairly easy to accomplish.
Making impact
(Force yourself into the vents)
Possible to miss

Dark world only. In the Lab, use a book to break the fan so you can enter the vents. You may have to throw the book more than once to get this done. Once the fan falls off, enter the vents. (Thanks goes to rogermorse)
Floor is lava
(Get into the lab when the floor is still wet)
Possible to miss

Enter the Lab without drying the floor, but be careful, the floor is electrified. I found that the best way is to run and jump on top of the blue fertilizer bag that is about half way to the door. From there, you can jump to the door.
Taping Killed Music Industry
(Listen through all audio tapes in the light world)
Easy to miss

Find and play all of the tapes in the light world.

Easy to miss

This achievement is easy to miss but easy to obtain. Find the memory book located in the Lab, take it to the incinerator, throw it in, and purge it.
The Gamer
(Get 300 points in RIESA)
Hardest achievement in the game

This one is self explanatory and in my opinion, is the hardest achievement in the game. Score 300 points on RIESA (in the Study Library). This is much MUCH easier said then done. My highest is 214. If anyone has any pointers, I'd love to hear them.

Okay, thanks for commenting. I always had all of the keys in my inventory before using them on the door. I'll make the changes and thanks for pointing that out.

Looks like you can miss the "key account manager" (three keys) if you put in two keys to their places before grabbing the third one from the rail car so they are not all in the inventory at same time.

Thank you for the compliment and thanks for the tip. I did raise my high score from 198 to 214 but it's a far cry from 300. I tried several times but I have to resign myself to the fact that this is an achievement that I'll most likely will never get. Nevertheless, thanks or commenting, I appreciate the help.

nice work, thx. in case you didn't achieved "the gamer". I found it very easy to do it like this:
stand on the left side of the screen. you gotta make it so that you will get kind of a "3d" view of the game, imagine like a tunnel/tube seen from the left side.. and the yellow stars coming from the left to the right towards you. then keep your plane always in the middle of the tube (never turn around the 3d tunnel completely). then just move up and down to avoid the stars. it's like jumping over it and going under the stars, concentrate on only three lanes and only the stars coming in the middle of the tube. sorry for my enlish it's not my mother tongue, but I hope you got an idea of what I mean. Just get the 3d view of the game (standing on the left of the round screen) and it's much easier like this instead of standing in front of the screen.. hope this helps
