As the description says you must collect the first start.

Can you find every star in Deep Horizon?
For this one you must collect all start, but atm the game has a glitch that, if you collect all 3 stars in the first level, you will unlock this one. So just collect them all on first all and you will unlock this achievement.

Decadethon
Your tenth death! We didn't you know you'd like it so much...

Fifth Death!
Getting to like it, aren't you? Your fifth death!

Just die 15 times and you will unlock it all :D

Hi, I've seen the glitch, and think I figured it out too - fullscreen runs on either Big Picture or 1080p exactly. Fixing it as we speak, should have a hotfix up shortly.

It seems your update destroyed the game. I can't play the game - I just see a picture of water and my cursor but can't do anything. And I hear music. Thats all.
I restarted the game about 20 times - nothing!
Redownloaded the game and verified the files, as well - nothing!
Restarted Steam - nothing, as well!
