fixed tiny brained math problem on vote ratio

• renamed sh_mvp_rtv_vote to cl_mvp_rtv
  
• renamed sh_mvp_rtv_unvote to cl_mvp_unrtv
  
• added sv_mvp_use_ulx_commands
  
• added sv_mvp_use_chat_commands
  
• rewrote the RTV module to work on x86-64 (not sure why this worked at all before, tbh)

relocated CVARS to have better timing with specific server types

added whitelist/blacklist
