Made it so that when you exit or die inside Charlotte's Phase Beast Lava challenge, you are teleported to Minehead instead of Sundered Battlefield

Removed the chances for the following affixes to appear on weapons:

  • Percent chance to burn
  • Percent chance to freeze
  • Percent chance to shock
  • Percent chance to poison

Removed the chances for the following affixes to appear on weapons and armor:
  • Percent fumble recovery chance
