It replaces the crowbar for both deathmatch and singleplayer.

Actually the green stuff..Remember in HL2 the medkits and little ones had green stuff. Maybe its a sort of gel that heals wounds in a heartbeat.

Nothing comes out of the syringe, so I believe you are just stabbing the enemys. Maybe that green stuff in the syringe is AIDS.
