That's a bug that occurs when NPCs don't use the default HL2 weapons, i.e. using custom SWEPs. Not much I can do to fix it.

I love your insurgency player model packs I use the heavy one a lot. Though there is one problem, they don't know how to use the melee on stock hl2 weapons. When you get close to them, instead of batting you in the jaw with the weapon they just to the stock idle pose.
