You need subsribe to all 3 items, wait untill download finishes and launch the game.
Then do nothing for about a minute, giving some time for mutators to be installed. You should see white text in the top right corner of the main menu, which indicates install process.
After instalation is complete, you need to restart the game. Then mutatots will appear in the list.
I know this is weird, but that is the way KF handles workshop items.

I subscribed and downloaded these packs however I have no doom 3 mutator and nothing has changed. Is there an aditional step that I need to do?
