I'm just too busy with other things. I miss mapping for the source engine, but I've got other projects taking up my free time.
I used to love taking requests and commissions, but I've been unable to make time for my own projects, let alone others. So I'm sorry to say I can't take your request. : (

Sorry, I'm too busy to make a walkthrough of how to specifically make a map like this, but I can at least break down the steps for you.
1: Acquire a model to be used for the map (Models Resource for example)
2: Using a 3D modeling program, create a collision mesh that matches the model. (This will probably take the most time and effort)
3: Export the model with your hand-made collision mesh into the Source Engine
4: Using Hammer, center your model in the map and position all the player starts, create skybox, add music and other fun effects.
5: Compile your map and test it out!
This map was one of the easier ones to do thanks to it's simple shapes. If you're new to modding for the Source Engine, I made a small series of simple lessons to help people who want to get into modding.

BTW from what I've seen, if I recall correctly, it's easier to import them as props than maps...
