uninstall it, start isaac, install it, delete the file grabbag then start isaac again. should be only sackhead now without the grabbag @crotchmonsoon

I am pretty sure he is looking at the orange pickup sack and doesnt recocnise the cat style with it. I don't like the mystery bag in neko style either. but I like the sack head itself

Nope, no other mod that modifies sacks. I'll try reinstalling it to see if it fixes it

I reinstalled the mod and it works fine. Are you sure you don't have a mod that modifies anything sack related, like a .anm2 file? Have you tried reinstalling the mod?
