Alright, so I experienced this first hand before and after you reported it, I began investigating. It's clear that the muzzle attachment bug and the c_hand issues happen when a barrel attachment for this gun is installed.
First of all, I assume these issues are caused by gmod being gmod, these should never happen.
The muzzle attachments being far from its place is caused by the combat recon, extended, and the tiger team barrel. Because of the new barrels differing in length, the muzzle flash bone and the muzzle attachment is moved accordingly, but for some reason, this change affects other weapons' bone vectors as well, causing the muzzle attachments on some guns to be misaligned. There is a fix to this in mind, but I need to keep investigating.
Guns affected: AK-47, Krig 6, FARA 83, XM4, MAC-10
And funnily, not every gun is affected by this one. Guns not affected: AK-47, Krig 6, FARA 83, Swiss K31, MAC-10, XM4, .410 Ironhide(WIP gun). Basically, guns made with newer methods do not have this issue.
There are also some other finger misalignments but they aren't severe enough to be mentioned.
This is the mother of all bugs I've encountered so far, definitely will take longest time to fix. Thanks for reporting.
Try telling your friend to turn off "Cheap Scopes" option in the User setting of ARCCW
