Simple, open the pak01_dir using GCFScape go to materials, vgui the scroll until you see idk Ellis for example, which is s_panel_mechanic, grab both the vmt and vtf files, this is the ingame photo extract it, make a folder, name it whatever, make a "materials" folder, then a "vgui" folder and put s_panel_mechanic.vtf file in the vgui folder, then open up vtf edit, import the photo you want to use, make sure to have it 64x64 (i usually use 128x128), save it as the vtf file you extracted, then find vpk.exe, this will put it into a readable mod then put your new vpk file in your addons folder and your done. If you want to toggle it on or off, add a addoninfo.txt file to the root of the folder.
