I noticed after the TLS update the arms on some of them (most noticably Ellis) have become messed up.

Yes. That is because the VPK is as it was untouched before the update. This was done on purpose.

So I dove into the VPK and it turns out there are no patch notes within, just a "thanks to" note. But by comparing the content and patch notes, its very clear that this VPK directly correlates to the version available on gamemaps that was released on 09/06/14, and replaced on 04/26/16. As for the workshop, the same version was released 09/06/14, but was replaced 03/25/21, much later than gamemaps, just as you said. So this VPK is definitely the version prior to the break, and was the 7th update to the campaign.
I did score a quick look at the VPK using GCFscape before uploading to the workshop, I think there was patch notes in the root directory in a normal text file. I'll definitely get around to doing that when I get home from work today. Very intriguing.
It could be the changes that were given to the campaign on Gamemaps in 2016, however the VPK's origin was the Workshop, 2014. So it depends actually if the workshop was updated first then they got around to updating gamemaps 2 years later, since this addon seems to have a history of sparse continuity updates between the workshop and its 3rd party hosts.
@RF Hard Rain: Downpour's Steam Workshop release did receive an update on 6th March 2021 after not receiving one since 2014 (Hence "Post-TRS"), but these might have been the changes given to the Campaign in 2016 as can be found for the GameMaps release: https://www.gamemaps.com/details/12237
Before their update on March of 2021 which made the campaign broken, the last update was August of 2014. That version ran from 2014 all the way until 2020-21, when The Last Stand update released. The resurge in attention to L4D2 is what sparked them to re-update the campaign. That is why I call it "Pre-TLS".
