Its incredibly common to use real gore for gore in video games, even today.

Valve stopped doing it around L4D2 and instead opted to use potato skins (which is why L4D2 infected dont look convincing) due to employees getting mentally disturbed from referencing the "Nightmare Folder" as its called.

tldr if you want to REALLY take the moral high ground just stop playing games with gore altogether.

In my opinion, the creepiest part of that original image isn't the mangled eye, it's the guy's hair. You get so used to seeing the HL2 model as bald that the frazzled scalp just takes you off guard.
