For example you could make it, that the doctor regenerates into the same version of himself. That would be very easy of course. You could also make it so, that he regenerates into random versions of himself. For example he could be transforming into the 11th, than randomly (when regenerating) into the 3rd etc, so that he randomly cycles through his former or latter incarnations.
1. While he regenerates he will burn a certain area around him, which could provoke more cautious play with him, because he could burn down the base for example.
2. Also the regenerating process could take a while and during it he could be vulnerable, maybe make it so that he only has 10 health during the reincarnation, and if he is than killed he turns into the ghost.
3. You could also make him loose half of his sanity or hunger or both after regenrating, as he turns into a very different version of himself and, as the 11th showed, gets very hungry.
4. You could make the tardis unacessible for a while after the regeneration, because it has to figure itself out as well. Or you could make the tardis break when the doctor regenerates, maybe giving back the most expensive materials, so that it has a cost to the regeneration.
7. Give the player an option to delay regenerating and give the doctor automatic healing during the delay (as with in the show), so that the doctor gets a chance to be very useful in boss fights, or to get away from the base to not burn down everything, but make him loose more sanity or something if he does that or make him slower.
Those are, obviously just a few examples, much more could be possible and it is entirely based on my opinion as a, admittedly, somewhat unexperienced player.
Didn't realize you lost sanity from killing things like rabbits until I was attacked by 3 birch tree guardians and was trying to defend myself. I had to run in circles until my friend could return because shadows were ripping through.
Way to go! No killing for The Doctor. Keep that peace streak up, no?
