Is there a mod to remove Iceland from the game entirely?
Iceland kinda kills the entire strategic element, when it strong arms the player to start in that country.

You can do it with custom scenarios. Create an event that sets Iceland's Dead population to an arbitrarily high value, like 1 billion. However, this will get your disease detected automatically.
