That is not exactly true. The chance of a 0% choice death / maiming happening is very, very high in the beginning of the game with the most junior characters. Still, in truth I prefer killing off characters if and when the opportunity poses itself. Suicide bombing IS a thing in this game B-)
Generally, it is "two strikes in a row and you are out."
A character may go down to 0 HP only twice in a single chapter before dying. If said maimed character got maimed, then healed up, the next time it gets the stuffing kicked out of it, it will die.
If you don't like parties full of peg legs and hook hands, it is simply better to dismiss them at the end of the campaign, or use them as support staff, exploring, cleaning up, patrolling and reinforcing.
And if you have the Town Market mod by smoker installed their value goes up even more - they may not be used again, but they can be left behind in towns with a market to act as brokers, trading resources to ensure your frontliners can upgrade their armour more quickly too by visiting the nearest town, as opposed to having to travel back to the market. But why would you want to do this, when there are so many options out there to prevent limb loss, or to replace lost limbs with something else?
