So the Workshop is pretty much just reskins?

No actually new weapons, items, characters, etc.?

Extremely disappointing.

EDIT: Is this because real mod support wasn't added until very recently? Was it not possible to do anything other than cosmetic changes before? With Synchrony, is it now possible to change and add gameplay-related things? Is there hope?

Such as? What are the "best" ones?

All I'm seeing are character/enemy/item/level skins and music replacement stuff. Seems even a lot of the bigger mods of this sort haven't been updated in forever and don't work with the current version of the game.

You need to access mod.io ingame through the pause menu inside of the customise setting. Here's somebody playing with a ♥♥♥♥ load of game altering mods: https://www.youtube.com/watch?v=5BX_JbGy5p8&t=1929s Includes new items (tonnes of items from Isaac, including a tonne of original ones), characters (over a 100 at this point I think?), enemies, and bosses (Marx, Galacta Knight, Clutch, Mask of Infamy, Rainmaker, and Loki).
