Have the following been taken care of (if only to rule it out)?

Are you following mod install instructions? Sometimes you cannot just sub to a mod. You have to install it. (IE Blue Shift mod).

Also run game verification with Black Mesa closed to see if it helps any here. You can do that before trying to get mods going in case you needed a clean slate of game files before you start modding them.

Yep sadly so. I even tried some other guides not on the community page. Seems dead.
