Modded items not working

Yeah, i need help. Im trying to get tarnished isaac from the epiphany mod, but the blighted photo dosent work. it also happens with other mods i have installed: Diplopia 2.0 and golden death certificate. None of them work, i play on repentance+, some help would be useful, thanks. (Im talking about modded active items)

To unlock Tarnished Isaac, you need to bomb a Treasure Room then bring the Blighted Photo to the Strange door in Depths/Necropolis/Dank Depths 2 and enter it and reach the end of Mausoleum/Gehenna 2 to grab the child's drawing and afterwards you need to find a key in home and open the chest to unlock it.

If this does not work you can always try to run it on just Repentance because some mods work better on Repentance then just Repentance+
