The clothes are still equipped but simply not visible, which means in the case of bra items or other clothing that constrain the character physics it would look a little weird as the breast shape would no longer be natural but compressed, for the sake of consistency all clothing items that do constrain the physics would need to be altered to remove that constraint.
This is a huge undertaking to not only edit the physics data but also change the transparency mask for every single clothing item, not to say there isn't a mod author up to the task but still it'd be substantial.
For the topic of the OP even though it was posted some time ago, I do not believe it likely for a mod to change the base game mechanics making clothing removed from the player inventory when getting damaged.
The reaper instakill looks good enough for clothes being removed and it works for clothes mods. Surely someone could figure out a way to utilize that to get a similar effect.
