More historical regional, centralised powers (MOD IDEA)
1. Certain cultures, like the Irish, Picts, Turko-Mongolic, West African (except Ghana & Kanem) and the Balts, Finnish and any other cultures that preferred centralised, regional autonomy over bureaucratic kingdoms are likely to try and fight for that autonomy.

2. Another way would be to make Kingdom tier titles get destroyed when the ruler dies, but passes down on a strong claim to his descendants.
