You are right with the adding of sounds to your preset menus, but pressing numbers while the UI is open will only change which preset menu you are viewing. (The number in the middle is which menu out of 10 you have selected, NOT at all the current sound number. You have 10 menus with 10 slots for sounds to be placed.)
You need to Hold R and hold mouse 1 or 2, then hover over a wedge that has a sound on it (the wedge will darken) and release either m1/m2 or R.
You can also press a number while playing to instantly change the instrument you are playing based on the numbers in the UI.

For example pressing 9 while playing w/ mouse 1 will change mouse 1 to "miku"

But numbers aren't at all required except for QoL features. Watch the video "Setting Sounds" above and if it's opening the spawn menu when you press Q, then it's 100% an add-on conflict.
You need to HOLD reload and HOLD either Mouse 1 or Mouse 2 and HOVER over a wedge (It will visibly darken) then RELEASE reload while being over it... Please watch the videos above in my post (especially "Setting Sounds"), you can clearly see I don't press the weapon selection binds at all (the numbers).
I do not understand what you mean by "sounds aren't being played." absolutely nothing has changed from how to play sounds you just press mouse 1 or mouse 2 and it should play the default sound without any selection. The only thing that changed is how to change your instruments.
There's nothing wrong with the addon, I've had multiple people test it out and figure it out on their own... you are doing SOMETHING wrong and I don't know what. I'll need to work on it to make it way more intuitive but past that I cannot help you out at this point, you are doing some step wrong.
The old version is available at https://codeload.github.com/CapsAdmin/wowozela/zip/5eb4bbd4e591f8ad62edbdd89abb0cc2ff04f059 if you really cannot figure it out but I will not give support for it.
What is your "wowozela_volume" convar set at? If it's set at 0 no visual or sound will appear.
You play sounds by pressing mouse1 or mouse2 with the menu not open at all??? It should simply play the default "bass" sample when you press mouse1 or mouse2 with a fresh wowozela.
