Report a translation problem
I promise I will let people know as soon as I believe I have a solution or explanation. In the meantime, I apologize for you not being able to play the campaign.
First, unsub in the Workshop (start clean). Next, went to l4dmaps.com, downloaded the VPK from there and installed it - note that it goes into addons directory not addons/workshop directory. Now, go back to the Workshop, and subscribe. You'll get a new file in addons/workshop (for me, 121086524.jpg, it may differ though), exit out before it finishes downloading. Move the UrbanFlight.vpk to the addons/workshop directory, and rename it as 121086524.vpk (match the number to the jpg if it differs). Log back in. L4D2 should now see you as subscribed, and have it installed. Presumably, everything tied together nicely so it stays in sync - fingers crossed.
One curious thing to note... UrbanFlight.vpk is 214M. The biggest VPK in the workshop is 199M. Wondering if you've gone over a limit and need to split the distro into 2 pieces (like many other campaigns have). Worth a shot at least...

I've also tried running l4d2 on a newer PC and get the exact same issue. The newer PC has generally high-end 2013 specs and Win7, whereas the older PC has XP and ~2007 specs. The way I installed l4d2 on the new PC was by copying my Steam folder (incl all my games) to it from the older PC AFTER I thought I had removed all signs of l4d2, then attempted to do a "fresh" install of l4d2 on it.
This is quite obviously an account/directory issue, and not the map.
Hope my long spiel gives some clues/insight.

These steps, combined with unsubscribing from the campaign in the workshop, should eliminate any trace of the campaign and L4D2's need to download it. This obviously stops you from playing the campaign, but I'd rather you be able to play L4D2 at all...
The only references on your computer to this campaign could be:
-- A VPK in the left4dead2/addons folder
It's possible (though unlikely) that you may have a package for urban flight in this directory. Delete it.
-- The workshop vpk and image in the left4dead2/addons/workshop directory
This would be two files: the vpk itself and a thumbnail jpeg. For me, the names are 121086524.jpg and 121086524.vpk. That might be the same for everyone. Delete them!
-- An entry in left4dead2/addonlist.txt
This is a list workshop items you have either subscribed to or downloaded (not sure which), and whether they are enabled in L4D2. Thus, there should be an entry with this campaign's corresponding number. It would look something like: "workshop\121086524.vpk" "1". Open this file in a text editor, and delete that line.
