First off, I wanna say to the creator of the mod, you did an absolutely fantastic job with this mod. Its pretty much what got me to playing Hard Mode a lot more because the new patterns and attacks adds a lot more complexity and feeling of fighting the boss and makes Hard Mode much more fun to play, and I wish you the best of luck with future updates to the mod because its just fantastic so far.

With that out of the way though, I feel like the mod really needs a bug reporting page to help take note of bugs or glitches that might crop up, so that's what I'm doing now. To anyone else, if you come across any glitches or bugs in the mod, feel free to use this place as a bug reporting area so they will not clutter up the description page with reports that might be cleared up in later updates. I will just note that I'm just a fan of the mod, and I felt like making a discussion here would keep the description comment area a bit clean of reports that would most likely get outdated by future updates and give reports a place to go to for easy searching.

Well, to start this off, in the most recent update today, I ran into a bug where during the Mom fight, she made the sound that would signify her about to drop a knife on you, but it never happened. Otherwise, the fight worked as intended, barring an attack that wouldn't work.
Well, since this place exists, I'm gonna report a bug or couple too.

A couple attacks (Mom's knife tossing and super ground stomp, Hush's vein pop attack) will sometimes be done at the upper right corner of the room instead of where they should be.

The mod conflicts with the "Harder Bosses" packs in that having that one installed will break many of the bosses here. Force them to do one attack over and over, removes the random spreads (for instance, It Lives has a final phase where it throws bullets all over the place while firing lasers. With bost mods enabled, the bullets all go in one direction as a continuous stream)
EDIT: Seems this was fixed. Only conflict I see now is Harder Bosses is doing something weird to ??? and making him follow vanilla behavior with all shots homing.

Seperately from that, I've had a number of crashes related to Mom. She does her laugh, like she's about to do the central stomp with the shockwave, and then the game stops. This may be another conflict with another mod I don't know about yet, or something broken in a recent update, because it only started happening in my last few runs. There's also some crashes I've had entering Delerium's boss room, but they aren't consistent and I don't really know why they happen.

Latest update completely broke the mod for me, none of the bosses are using any of their new attacks and all of the new enemies/entities just freeze and slide around the room.
Edit: Fixed on January 22nd

Mom keeps crashing the game whenever she decides to use a new attack, not sure why since it works fine if I just use the console to go to Depths II.

I'm not sure it this could be considered a bug or bad design decision, but I'll post it anyway:

Delirium's knife abomination attack (where it summons this big knife-like thing to crash down at you), I swear is impossible to avoid taking damage from, since the thing simply hits you with no chance to react or avoid it.

It may also be that I don't know how to avoid it, but I'll still post it here to learn of a way to handle it.

The enemy's called Memories III, there's 2 versions of it according to the console, the normal version and the one tagged with (Disposable).

and yep (Disposable) is impossible to avoid, probably a bug, as I don't think this happened before.

Oh boy, another bug report:

While Hush is dying, spiders can still spawn from its veins, meaning that you'll have to kill them yourself, and that's no fun.

Something about Hush seems to crash the game now. Whatever move he is supposed to perform when on the stage where the hands spawn in just ends up crashing the game.

When Delirium transforms into The Lamb, and is under half health, he has a tendancy to... leave his body behind after Delirium transforms again, staying in the room and continually attacking and moving around. You can kill it and it'll stay down, but adds unnecessary difficulty and clutter to the room... Especially when there's more than one at once. And the bodies stay on the floor after death, and ALL come to life again when Delirium transforms into The Lamb again. I think the game randomly sometimes tries to clean this up as there are times where the bodies all randomly dissappear, but it isn't consistant. Just makes the fight harder than it needs to be when The Lamb is in the transformation pool.

Can confirm with Alien, when I was fighting Delirium, I encountered a crash during the pit phase. I heard a whooshing sound, and then crash.
I don't know if I should post this here, but since there isn't a suggestion table, I'm gonna put it here because why not:

The big Brimstone beams that constantly rain down on you while you fight Delirium can block a big part of the screen if the beam is fired at the bottom of the arena, meaning that you won't be able to see bullets or anything else coming at you.

Also the Memory III still does unavoidable damage to you unless you manage to kill it before it goes off to attack you, with this issue now being worse thanks to the Brimstone beams.

Tried Delirium out to clarify with Nuclear. Can confirm this is indeed a problem. To add, Isaac's phase 1 patterns, the box pattern and the spinny tornado type pattern occur too quickly. Normally Isaac during phase 1 has a tell to let you know an attack is being performed. The new patterns however don't have such a tell and he'll do the attack instantly, which can lead to damage that couldn't be predicted as the box hits very quickly in particular.
