To solve the missing mask problem you need to have an invisible head equipped in the face slot, the mask is apart of the uniform equipped on the Torso (Helghast body).

Now it took me a long while to find where the uniform was located because it blends in with everything else. it can be found in the Torso tab when customizing the body.

I had to resubscribe and after I played a mission I found it.
