Detailed Guide
Here's how to reliably use this mod.

There are 2 dialog options to use at Plastic Surgeons, both of which will cause the surgeon to follow the character that talks to them and trigger the character modification screen periodically. How and when that screen is triggered, and for what character, depends on which option you choose. Any plastic surgeon should be able to do this (I used one named Doctor Willis at the Waystation in Grey Desert), but I haven't directly confirmed that.

Option 1 ("I want him to that"): Setup

This option targets the left-most character in squads regardless of that character being an animal or not. There is a charge of 200 cats per character when using this option.

1. For each character (animal or otherwise) that you want renamed, create a new squad and put that character in the squad. Since only the left-most character in a squad will be targeted by the surgeon, single-character squads is the easiest way to use this option.
2. Move each character to stand in a line, spaced out roughly 1 stormhouse apart.
3. Move a character that can initiate dialogs and that is not in the left-most position of a squad to the plastic surgeon.
4. Start a dialog with the surgeon.
5. Choose "I want him to that". The Plastic surgeon will say "Where is that person".

Option 2 ("Renaming Animals"): Setup

This option is exclusively for animal characters. There is no charge for this option.

1. Create a new squad and put only the animals you want renamed into it.
2. Move each animal to stand in a line, spaced out roughly 1 stormhouse apart.
3. Move a character that can initiate dialogs to the plastic surgeon.
4. Start a dialog with the surgeon.
5. Choose "Renaming Animals". The Plastic surgeon will say "Which animal?".

Option 1 & 2: Execution

After going through the setup steps above for whichever option you want to use, the rest of the process plays out the same way.

1. Move the character that started the dialog over to the first character in the line. The surgeon will follow the dialog-starter.
2. After about 15-20 seconds of choosing the dialog option, the surgeon will say "{character name} renaming" for the qualifying character that they are standing the closest to.
3. After a short delay, the character modification screen will activate. Make your modifications and click Confirm.
4. Re-select the dialog-starting character.
5. Move the dialog-starting character close to the next character in the line. The surgeon will follow the dialog-starter.
6. After about 15-20 seconds after confirming the last modification, the surgeon will once again say "{character name} renaming" for the character that they are standing the closest to.
7. After a short delay, the character modification screen will activate. Make your modifications and click Confirm.
8. Re-select the dialog-starting character.
9. Repeat this process of moving the surgeon to the next character in line and making modifications.
10. After 1 in-game hour has passed since you first chose either option, the surgeon will say "bye" and walk away.
11. If any characters still need to be renamed, talk to the surgeon again, choose the same option you were just using, lead them to the next character in line, and repeat the process.
12. If you wish to end the process before the 1 in-game hour time limit is up, talk to the surgeon and choose "thats enough". They will say "bye" and walk away.

Notes

Save your game before you first talk to the surgeon.

There is a 3rd option, "[...]" which seems to trigger the same behavior as "I want him to that".

If you speak to the surgeon mid-process (say, to stop it early), avoid choosing the "[...]" option. It will trigger the "I want him to that" option, even if you had originally started the "Renaming Animals" option. Avoid speaking to the surgeon mid-process, unless you are certain you want to end it early.

After telling the surgeon "thats enough", sometimes, if you talk to them again, they will still have the "thats enough" and "[...]" option. The "[...]" option will make the surgeon start the "I want him to that" method again, but they may keep walking away. Avoid ending the process early, unless you are certain you will not talk to the surgeon again for at least another 20 minutes.
