Medicine could be related to growth, where you're rated on overall population growth within a certain time frame.
Medicine would be food produced, housing and maybe Science?
1st Place: 20% Growth in all cities. Additional Housing or other yields towards sewers.
2nd Place: Eureka and Science Buildings provide Housing
3rd Place: All cities are provided with +2 Housing
You could change these around of course
Science, Great Scientist Points, Eureka earned (give out +10 score or something similar?)
1st Place: Great Scientist, +2 New Techs and gain Science when getting a eurkea
2nd Place: +2 Eurekas and Science Buildings cost one less Gold to maintain
3rd Place: +1 Eureka and Bunch of Great Scientist Points
1st Place: +1 Science and +1 Production from Iron, Niter, Coal, Oil, Aluminium and Uranium resources
2st Place: Great Scientist
3st Place: Great Engineer
Economic:
1st Place: +2 Science from Markets, Banks an Stock Exchanges.
2st Place: Great Merchant
3st Place: Large sum of gold

Earn Points by: Completing the 'economic investment' project, earning Great Merchant Points, and having high ammounts of Gold Per Turn.
Gold Tier Award: 1 Great Merchant, 1 Unique Luxury you can only get by winning this competition, 2 civic boosts, 1000 gold
Silver Tier Award: 1 Civic Boost. 500 gold
Bronze Tier: 1 Civic Boost. 250 gold

Chemistry and medicine could just be flavor variants of the physics prize. Economics technically isn't a Nobel Prize anyway.
Since the competitions are now universal and no longer Sweden-specific, I think it would be more interesting to look at other awards: Fields Medal, Oscars, etc.
Or those could be alternative names for the competitions whenever Sweden is not in the game.
Literature Nobel Prize becomes the Oscars. Physics Nobel Prize becomes the Fields Medal. Peace Nobel Prize becomes a competition for Secretary General.
