While a player is using an Outfitter playermodel:

Shooting will cause a 1-second T-pose failed animation.
The reload animation will just T-pose.
Depending on the model, having the safety on will cause a T-pose.

Also, some models using Outfitter fail to show the weapon in the model's hands, the hold type is correct, but there's just no gun.
