Firstly I'd like to thank you for taking the time to write up this feedback, its very gratifying knowing someone likes my item enough to spend time trying to help me improve upon it.
just to respond to some of your points
1. While I certainly see the similarities between this and the Gothic Guise, I'd say the resemblence is fairly minimal, you have to bare in mind that I made this item with compatibility in mind, so stuff such as eye placement remains the same as the default mask and many mask replacements respectively.
2.The more rounded snout is certainly a nice design, but I wanted to stick with a more whimsical fantasy dragon rather than something more grounded in reality. So i've gone for a more pointed snout, its more in line with the type of dragon illustrations I'm fond of, so its more a matter of personal taste.
3.While adding spikes and spines to the top of the head would make it more distinct, it would sadly make the item work much less effectively with pre-existing hats and other cosmetics, this is why i've positioned the spines / spikes where they currently are, to maximise compatibility and to reduce clipping or atleast prevent clipping that looks too dreadful.

it does fit the art style of tf2, but it doesn't fit the "theme" of tf2 per se.
