when can I expect to see the first custom enemy?
I started a new game with only this enabled (from chapter 2). I am now at the first dead combine soldier (just before the dark part with all the barnacles) and I havent seen any of the new zombies yet. where are they?

It's right after you see the dead soldier. You'll see the first zombie equipped with grenades in a door after the hallway with hanging soldiers.
