It'd be cool if black markets didn't invalidate angel rooms too! That might be a conflict-y change to implement though.
Unfortunately it's hard coded in the game that black markets count as devil deals, thus rendering angel rooms moot. And as far as I know, there's no way to modify the angel room or devil deal chance using the modding API.
Too bad about the angel deals. I've always liked their dynamic, and I think it could've been cool if you could take them and still sac some health for offence in the form of black markets. Oh well!
Since the reroll strat only works mid game, I suggest replacing the lost's holy mantle with a wooden cross when you choose to start d4ing if you want to choose at all. The small safety net of wooden cross combined with the really good transformations should make it balanced. Additionally, this way the early game can stay in tact (cause you still have holy mantle for it), and you can choose to gamble your run or not.
Replacing her passive trinket purple heart with crow heart could be a devilish way of undoing this synergy. You'd take eternal heart damage the first time on a floor, and not lose devil deal but lose your red heart container generation. However it would synergize with yum heart, because yum heart generates 2x as much red hearts.
Maybe this is too different and would be more suitable as its own mod, but regardless, great mod! I love the balance changes and hope you keep expanding on them into perhaps the rest of the items.
EDIT: Oh shoot I hadn't realised you had an items reforged mod! Well then, I change my last statement to "I hope it gets updated for repentance" :)
Yeah I now realise such an addition would be far mor suited for that mod rather than this character focused one.
Isaac:
I don't feel Bag Lunch is thematically appropriate nor does it deepen his gameplay. With the repentance update, I'd say give him Dice Bag so he truly becomes a character about manipulating runs to your advantage. If it feels a little excessive on the dice play, then the new Lucky Sac trinket could also be a good idea to ensure Isaac has a safe economy of items (as the 'beginner' character) which can later easily be replaced for something significant to a build.
EDIT: I realise Bag Lunch is actually quite alright. Though I do like Lucky Sac as a starter trinket, it might be more suited to someone like Cain, so Bag Lunch is the way to go unless Dice Bag proves preferable, which I think it might!

Eve:
You should replace Daemon's Tail in favour of Eve's Bird Foot. It makes a lot more thematic sense, and even if Daemon's Tail works as protection to your babylon state early, it can be make things too easy. The bird should ideally be able to supplement your damage where it might be lacking earlier on, to then gradually get to a stage of having enough soul/black hearts to protect your babylon state.
Samson:

Azazel:
Both trinkets aren't great for Azazel, and just feel like buffs rather than features. I'd say Daemon's Tail feels a lot more appropriate here (he is a demon after all), so I would grant him this as his effect. As this will be a fairly powerful effect for Azazel, he could start with the drawback of a permanent trinket - Tick - loyal to the essence of heavy damage but restrictive. If it is too restrictive, a different idea could be the new trinket Wicked Crown (or Bloody Crown for something more generally useful but consider his default power in this case, don't recommend this).
Lazarus:
Rosary Bead doesn't feel appropriate. He's not a character heavily focused around angel deals, and many use devil deals to get one last item that then kills him for the buffs (not sure if this can still be done in Repentance given devil deal updates). Regardless, you usually still want to expend all your hp on something useful and devil deals are the way to go, so instead I'd make him start with Bible Tract so that the value of eternal hearts becomes a large part of play (given his health reset upon death).
Eden:
Practically a given, no complaints here.
The Lost:
Very smart pick with Faded Polaroid, given that he's a ghost. Now, with the new trinket Your Soul, I would give him that as his starting trinket over Fragmented Card, makes that first devil deal you take much more important as you can get two items (if this works on The Lost) and is obviously thematically appropriate. As he no longer has the D4 with Repentance, the Holy Mantle rerolling issue is now obsolete.
Lilith:
The Blind Rage trinket is actually a brilliant pick, very smart. With the new trinket Adoption Papers now available, I'd say that's a more suitable pick that Child Leash is. Truly build a familiar army! However I would make Blind Rage a permanent effect, and Adoption Papers the starting trinket.
EDIT: I would actually make Child Leash her permanent effect over Blind Rage. Even if Blind Rage is a cool pick, Child Leash helps make her more playable and fun so definitely stick with that feature.
The Keeper:

Apollyon:
I would say now that Tainted Apollyon exists, save the locusts for him as he is evidently locust-based. Also, Callus makes no sense to me for his character. Given his little wings, I think his innate trinket should be Bat Wing. As Apollyon is a Fallen Angel, he should start with Rosary Bead, allowing for more angels rooms that have more potential with The Void than devil deals.
The Forgotten:

Lastly, to balance the Tainted and Original characters, would it be possible to add in the option to make the Original characters' active items (for those who have one) permanent like those of the Tainted characters?
That's the feedback, I'll probably be writing my suggestions for Bethany and Jacob & Esau now, then all the Tainted characters. Take my feedback/suggestions for what you will :)
Bethany:
Innate Trinket - Mom's Pearl (EDIT: Expansion Pack)
Starting Trinket - Duct Tape (EDIT: Mom's Pearl)
PLS DO NOT GIVE HER THE WISP TRINKETS (Beth's Faith, Beth's Essence). This is much too obvious, and would make her life much too easy. Her gameplay is all about maintaining wisps with what she has, and I assure you, she has plenty. She is also very powerful, hence simply Duct Tape as a starting trinket for consistency + Mom's Pearl as charge support.
EDIT: Actually I don't like the idea of Duct Tape; takes away from her uniqueness. I would recommend the new trinket Expansion Pack instead, making this her innate and Mom's Pearl her starting one. It simply gives her a chance of getting two wisps instead of one.
Jacob & Esau:
Innate Trinket (Both) - Sigil of Baphomet (can also consider Blind Rage)
Starting Trinket (Jacob) - Cancer (can also consider Bright Bulb)
Starting Trinket (Esau) - Curved Horn (can also consider Dim Bulb)
Another idea I had was simply giving them both the same starting trinket - Found Soul. But that sounded too meme-y, so I came up with this. The new trinket Sigil of Baphomet on both characters just helps you take less bad damage through momentary invincibility, therefore making the characters a little more usable. Cancer to reinforce Jacob's focus on Tears. Curved Horn reinforces Esau's focus on damage. The reason for the OP trinkets? These bad boys are in desperate need of actually being winnable given their current options. I might have overdone it here, but then again it might also be just what they need. The character's essence is still intact (i.e. these trinkets don't take the easy route of giving them a bajillion health options, cause that's their usual weakness since you need double the hearts to account for impossible controls basically murdering what you do get) so this should allow them to genuinely burst things down between the two of them, fulfilling their roles for victory. The bracketed options are other trinkets that make them a more active character to use, as food for thought for you to balance.
I will probably do my Tainted character suggestions tomorrow, but I do hope my suggestions genuinely help.
Tainted Characters Suggestions:
Isaac:
Innate Trinket - Telescope Lens
Starting Trinket - Missing Poster
Given his limited space, Telescope Lens can help enable Isaac to pick up ‘high value’ items through the Planetarium, while not regretting skipping item rooms given his limited slots. Missing Poster is thematically appropriate as Tainted Isaac is the beat up, hidden in a closet Isaac; the one who’s missing.
Magdalene:
Innate Trinket - Bloody Penny
Starting Trinket - Mother’s Kiss
Mother’s Kiss grants her an additional piece of optional health that has gameplay considerations given her focus on taking and dealing damage, all while not making her op in the slightest. Bloody Penny makes collecting coins much more interesting for her, allowing her to drop hearts that she can collect at her own leisure to procc her creep if needed while the design of her character doesn’t allow her to overheal. Do not give her Mom’s Locket, this is much too op given her already enhanced healing.
Cain:
Innate Trinket - Lucky Sac
Starting Trinket - Bag Lunch
Lucky Sack enhances Cain’s resource accumulation and allows him more crafting considerations given the limited number of pickups that drop each floor. Bag Lunch mainly because it’s a similar sac-like item, and further allows him to collect pickups. Ideally Bag Lunch would be given to him, while untainted Isaac has Dice Bag.
Judas:
Innate Trinket - Black Lipstick
Starting Trinket - NO! (EDIT: do not give him NO!)
As Judas is unable to gain any form of red hearts, Black Lipstick helps him gain the black hearts he needs, and black is just his theme. Finding another balanced trinket for Judas is difficult, as Dark Arts can provide him with huge buffs. So, NO! felt appropriate so he is incentivised to stick with it while reducing the number of ‘useless’ items he might get.
EDIT: I realise I'm an idiot. The new active items are an innate part of their kit now so NO! would be a terrible items to give him.
XXX:
Innate Trinket - Broken Ankh
Starting Trinket - Gigante Bean
Given his very poopy nature, the new trinket Gigante Bean provides an obviously related power. Then the Broken Ankh just for a chance to turn back into the original Blue Baby, again, related to him as a character. Brown Cap would be WAY too op, do not give him this.
Eve:
Innate Trinket - ?
Starting Trinket - ?
Do not give her Lil Clot, her gameplay is all about generating her own clots so this is unnecessary.
I honestly can’t come up with anything for her. I might come back and edit this if I do, but for now I’ll skip her.
Samson:
Innate Trinket - Swallowed M80
Starting Trinket - Walnut
Swallowed M80 synergises well with his melee capabilities if you do happen to take damage up close and personal, simply reinforcing how berserk his character is with some war veteran vibes. Sticking to the theme of explosions, Walnut can grant Samson a few more consumables, and should procc through Swallowed M80. Red Patch is another item I was considering. If you do decide to give him Red Patch, be very mindful of balance.
Azazel:
Innate Trinket - Bat Wing
Starting Trinket - Wicked Crown (again?)
Bat Wing; give him back his wings! Azazel is just as strong as ever, so Wicked Crown could once again be a fair item to give him. Restrictive (and replaceable) but still has potential use, and definitely thematically appropriate. Pls do not give him Azazel’s stump, it only takes away from his uniqueness.
Lazarus:
Innate Trinket (Alive) - Watch Battery
Innate Trinket (Dead) - Devil’s Crown
Starting Trinket (Alive) - Blessed Penny
Starting Trinket (Dead) - Cursed Penny
Devil’s Crown as an innate trinket makes the changeable item room (based off what character you are) a gameplay mechanic that makes balancing the characters more interesting, and provides a regular chance at powerful items. Furthermore, with Cursed Penny, Dead Lazarus reinforces the theme of the devil, and can prove useful in a pinch where you can’t access certain rooms (secret, shop, treasure, etc.). However, the holy counter to this is the Blessed Penny which will be a huge source of health split between the two characters. And lastly the Watch Battery just helps charge up Flip so it can be used more flexibly.
Eden:
Innate Trinket - Modelling Clay
Again, this one is another given. Just a slightly more exciting Error that makes his random passives even more random!
The Lost:
Innate Trinket - Wooden Cross
Starting Trinket - Endless Nameless
The Wooden Cross would help The Lost feel much more fair in terms of consistency, while holy cards are there for desperate times. Endless Nameless helps support him in obtaining more Holy Cards and may prove lifesaving.
Lilith:
Innate Trinket - Umbilical Cord
Starting Trinket - Nuh Uh!
Umbilical Cord for evident thematic reasons. Nuh Uh! also works for thematic reasons as you manipulate drops past the womb, like manipulating your womb with C Section. Simply useful but not really a crazy buff to a solid character.
The Keeper:
Innate Trinket - Swallowed Penny
Starting Trinket - Silver Dollar
Swallowed Penny is designed to give The Keeper a chance to regain health against bosses that do not spawn mobs. The Silver Dollar gives The Keeper more chances for shop plays, and is obviously thematically appropriate.
Apollyon:
Innate Trinket - Cricket’s Leg
Starting Trinket - Apple of Sodom
Inspired by the theme of locusts apparent in tainted Apollyon. Cricket’s Leg offers a chance to increase your damage through such locusts, and Apple of Sodom turns red hearts into a greater gameplay consideration. Do not give him Apollyon’s Best Friend, he’s got Abyss to basically do that.
The Forgotten:
Innate Trinket - Hollow Heart
Starting Trinket - Polished Bone
Sticking to the theme of bones, Polished Bone allows The Forgotten to have a semi-reliable ranged damage source while getting a few bone hearts along the adventure with Hollow Heart that help enable his risky in-and-out playstyle.
Bethany:
Innate Trinket - Beth’s Faith OR Beth’s Essence
Starting Trinket - Duct Tape (again)
Beth’s Faith OR Beth’s Essence allows Bethany to get some of her good old original wisps with the same risk-of-loss mechanic. Giving her both on top of her new passive wisps generation would be too much and takes away from her new wisps (called “ghosts”). So, Duct Tape would again be suitable for Bethany as it can grant her greater control of her ghosts’ positioning, to be less at risk of losing them.
Jacob:
Innate Trinket - The Lighter
Starting Trinket - Found Soul
Given Esau’s fiery demon theme, The Lighter feels like an appropriate fiery addition and helps this risky character clear floors faster. Found Soul gives Jacob back a partner to control, and invites caution in Jacob’s play for a reward.
Sorry for all the writing, but I do hope you give it a good read as at least food for thought. Honestly I’m sure you already have a plan for all of the new characters anyways, but if my lists have helped even make one revision, then that’s all good in my books :)
