A lot o' things, start out by putting it in-game by using the Team Fortress 2 importer.

Try giving it an actual texture instead of just ugly base colors. Don't use that bright blue, it looks really, really bad. I can't really say anything on how to texture, there should be guides on it though. Look at some hats like the backwards ballcap for inspiration on how to texture it.

well.. first you can start by improving... EVERYTHING! take the cigarette and the necklace off, use actual textures, not base colors, change the name, make it smaller... the list goes on..
