Design wise he would be even more decomposed to the point that even his other eye no longer works. Becouse of this he can no longer fire tears but he has another way to attack, first is that every now and then he produces blue flies (up to 5) and the second is a poison aura that surrounds him (alternativly instead of an aura it could be a poisonous breath that's kinda like Soy Milk on Azazel's Brimstone). The poison damage given to the enemies by the aura is influanced by his attack stat, the size of the aura depends on your range and fire rate determins how quickly the poison deals damage (If it's a breath instead then it would be mostly normal).
The original design for Cadaver was actually a character that fired blue flies instead of tears. That design didn't feel great to play though. You couldn't break fires, poops, or TNT. That could be partially mitigated with some alternative attack, like the ones you mentioned, but I ended up scrapping it for the current version of Cadaver.
I have some new plans for Tainted Cadaver now though. The overall goal is for friendly units to be at the core of his playstyle while still retaining the ability to shoot some sort of tears. Your ideas aren't far off things that I've considered myself or things that I'm playing around with right now.
Whatever design I settle on, Tainted Cadaver seems like he's going to be significantly more complex, development-wise, so he might not be here for a while.
I agree, making him depend on shooting flies might be a not very fun idea, this character is one of the best out there, it has everything a modded characters should have and even fit the lore, I have good expectations for Tainted Cadaver and please make him as fun and interesting to play as the original but of course with that "tainted" touch :D
What about drawing some idea from Melt (from Nuclear Throne)? Not the corpse exploding part as it has been implemented as Vade Retro
but I think you mean Ipecac?
Tainted Cadaver is very strong in normal rooms, and is considerably easier to play than the normal Cadaver, but there's still difficulty in his limited max health (4 bone hearts).
