If there are more people who want this feature then I will implement this. Just let me know how you want to see this implemented.
And do tell me what you enjoy the most about this mod. Which are your favorite options? Which are your least favorite and why? How often do you pick any of the choices? How well balanced do you think each of the options are?
I have done it by design that everything is english only, mainly because I don't speak russian ;p. It is possible, but then I also need to learn how to refactor the code and how to make translations files. Perhaps next time when I work on the mod and there is more interest to translate it into more languages.

Hey, this is a great mod!
I just want to clarify what I THINK @nerman8r is requesting:
When he says "allowing a Neow choice when playing a Custom game?", what is actually happening is: When you choose 'Modifiers'(Base Game Modifiers) in 'Custom Mode', for example, the "Big Game Hunter" modifier, you are no longer provided with a Neow option; i.e. After clicking 'Talk', the next option is 'Leave' without allowing any Neow choice.
EDIT: The problem seems to exist without even selecting any options in Custom mode; Ascension Level 0, no special seed, and no Modifiers. Only Standard mode works normally.
I believe @nerman8r wants a patch that does not disable the Neow option if any Modifier is active in the run. This behavior seems to be Vanilla behavior.
Currently, with this mod, 'More Neow' + the mod, 'Permanent Neow', we can have both Modifiers AND the 4 Neow options to choose from in every normal AND Custom run.
EDIT: The mod 'Filter the Spire' also has a similar function to 'Permanent Neow'. Using FtS rather than PN, allows using Modifiers alongside having a choice from the Neow options! I don't know how that mod does it but it seems to fix the issue for all modifiers except the "Heirloom" modifier. I guess that the rare relic you receive from the "Heirloom" modifier is kind of like a Neow choice in itself so I guess the vanilla behavior of that is fine.
2nd EDIT: To be clear; only 'Filter the Spire' works (not 'Permanent Neow') to fix all these issues.

Unless I'm misunderstanding what you're saying, I don't think this is right. I have used both mods and I still don't get the Neow options when playing a custom run.
Thanks for the bug report. I looked into this and turns out this is vanilla spire being spire. It will be fixed soon. Turns out an attack action can steal gold, but it can only steal gold if you don't have 0 gold. Having negative gold is therefore allowed to be stolen from. Now a looter/mugger can no longer steal from you if you have 0 or less gold.
Sorry if I wasn't clear enough in my edit: Filter the Spire's function works. Permanent Neow does not. Try that one out if you haven't

Thank you for the report. Spire defaults any new run in two options: Neows lament and +10% max hp start. Only when a run reaches the act 1 boss or is a seeded run will Spire generate the four options start. If you did this and spire still only shows the default two options then I recommend reaching out to me on the official STS discord.
Last point of convenience. There is a mod that always forces the game to start with 4 Neow options called "permanent neow options".
