The Door can be found in multiple spots around the garage (2 Spots inside, 2 out front)
or part of the buyable debris in the wooded path. (the $1250 one)
The dolly/handtruck piece can be found in 2 spots outside the shed containing speed cola, inside the cabin next to spawn, in the corner of the barn (as soon as you walk in, on the left)
Or just past the buyable door in the bunker, on the right as soon as you walk in.

First, turn on the power and open up the garage using the 3 digit code. The code is found on clip boards. Each board has a light next to it, indicating which color space the number goes.
Blue number - On the ground right by Quick Revive
Yellow number - Inside the cabin by spawn, on the wall opposite Stamin Up
Red number - On the floor in the stairs of the bunker

After you've opened the garage, make your way back to the bunker. Look into the bathroom (Last door on the right) You will see a light switch with 4 boxes for switches and a little red light flashing. Shoot the BOTTOM LEFT square of the switch.

Shooting this opens the door at the very beginning of the bunker, on the right as you're walking toward the exit.

On the ground, you''ll find a fuse. Pick this up, and head to the garage. As soon as you walk in the garage door, look left and find the fuse box on the wall. Place the fuse here.
Now make your way to the Speed Cola shed, and you'll see a new door has opened. Go inside and you'll find the buyable ending door.

After purchasing this door, zombies will spawn forever. You must now make your way around the map (the long way) back to the garage, and you'll see that a snow bank has been cleared and you can now escape to the truck in the distance.

PHD and Widows are in the secret rooms you go to while teleporting.

The 3 Teddys can be activated to play an Easter egg song. The song is Shaking Cages by Silent Theory.
Bear 1 - In the cabin right by spawn, sitting against the mattress on the floor.
Bear 2 - Behind the bunker entrance, next to Deadshot between the rocks.
Bear 3 - Upstairs in the garage
