Crash chapter 2 + temporary solution
In chapter 2, if you're going to that hole, you will get crash game
reasons cause crash is from entities were not precache from infront window

the solution only is look at the floor, go backward, avoid window entities, once you're jumped down and get away from that window (make sure you notify other teammate to avoid crash!)

then you can process to continued game without any problems

I did play the second map again today without it crashing. I didn't use this solution though. I typed "sv_consistency 0" into the console before I played the map. That console command might prevent the "A Dam Mission" campaign from crashing too. The last map of that campaign usually causes the game to crash to the desktop. There shouldn't be any reason to enable server consistency again. The game should do that after a restart. That's the game's default setting. A person could type "sv_consistency 1" into the console to enable it again..

I played the second map multiple times with server consistency disabled. The game did not crash once. I think it's best to always keep server consistency disabled if you never play online. Server consistency is necessary to make sure every online player is using the same files. There is a way to permanently disable server consistency. A person would have to create an autoexec.cfg file with the command "sv_consistency 0" and place that file into the CFG subfolder.

Local server + custom campaign always forced you to run sv_consistency 0, so it's not really a problem at all, the main reason came from entity were not precached, only solution is told the author to fix or use my temporary solution to bypass chapter 2

That's not true. Server consistency is enabled every time a person starts the game. I checked to make sure it was disabled before I started playing the second map. It wasn't. That's why I had to create an autoxec.cfg file. You seem to believe disabling server consistency had no effect. The second map did stop making the game crash to the desktop after I disabled server consistency.

I wanted to offer anyone reading these comments another possible solution even if you believe my solution is total nonsense. That's fine.
ofc sv_consistency 0 always gave effect to allow play custom campaign, i didn't said that not recommend to disable, i just explain about turn on sv_consistency 1 is an bad idea when play custom campagin, even you don't use any mods beside custom campaign

The author needs to fix this campaign so that the second map doesn't keep crashing. That would be the real solution. He obviously has a great imagination. He spent many hours creating this and can't find the time to fix his mistake.
