ACE is fully compatible and supported, though bear in mind that you would NEED to take some time finding medical setting that work for you as most people want different levels of difficulty from it.
AIS is not supported as far as I know.
We have had a wish for the SOG medical system, or something in that style, already and there was a discussion about it that ended up with us wondering how it could be balanced for play, can't exactly have you standing up every time you get shot.
As for the respawning when dying solo, take an ai with you and hide him somewhere out of the fight. His presence will keep the area loaded as you make your way back to the fight.
The AI difficulty settings in the setup gui are modifiers to the skill value of the respective sides. You need to pay for training to make a big difference in your AI's abilities.
As for the broken dragging, could you please join our discord and raise this issue there as that may be a big on our end that needs looking into.
When it comes to balance when someone is downed I agree they shouldn't just be able to get up. If you could crawl to a better spot that'd help immensely. I've taken a look at the SOG advanced revive module and there a bunch of settings on it. Things like roll over to crawl, crawl with weapon out, throw grenades while crawling, a timer of how long a person needs to wait while downed to withstand, etc. If it'd be possible to tune it through CBA that might help. At the moment I use ZUES to make my 3 team mates Medic, Engineer, and EOD.
My AI are level 10 from Petros and the only settings I've messed with for the difficulty is through the Initial menu that pops up. I should probably mess with the settings in CBA but I'm afraid of making the enemies too easy haha.

As for ace, there's a whole heap of tuning you can do to the medical system, I don't think you can go so far as you said but you can try to make players incredibly resilient to damage if you wish.
We are well aware of the SOG withstand system however we can't include it directly as that would introduce a dependency into the mission and we are trying to avoid that as best we can, plenty of players don't have the cdlc after all.
If you go through the addon options, you can disable everything besides the medical system. ACE is highly customiseable.
Diwakos Armor Plate System is imo by far the best medical system in arma if you want something more lightweight and arcadey vs something like ACE, PIR, etc. Since APS does not work for Antistasi i use Ace Medical without advanced mode and with a faster bandage mod i made and speeds turned up, it makes it alright at least, but the AI are really garbage as medics with Ace Medical and you can only really ever rely on humans getting you up.
Diwakos APS have good settings, solid AI revive, and feels like it plays well (unless ofc you purposely tweak it to be "op").
my biggest wish would be APS support for Antistasi
as it stands its actually not playable with antistasi at all, because if you go down and are revived, whenever you press "R" next you are slayed and respawned, as the antistasi respawn system lies active in the background.
